Cardiac neoplasms, whether primary or secondary, are more and more easily diagnosed in the present era. For the primary heart tumors, myxomas and sarcomas constitute the most common neoplasms, whereas for the secondary ones, lung cancer constitutes the most common primary location. Cardiac neoplasms may involve the endocardium, the myocardium, the epicardium, the valves or any combination of these. Their clinical manifestations are not specific and depend on their anatomic location and size. Diagnosis of cardiac neoplasms has become more feasible with the use of echocardiography and other imaging techniques (computed tomography and magnetic resonance). The major problem, however, still remains that most diagnoses are late, especially for malignant tumors. For resectable tumors, surgery remains the mainstay of treatment, whereas for metastatic disease palliative treatment remains the only therapeutic option. This paper reviews the types of heart tumors, their clinical symptoms, the diagnostic approaches and the therapeutic tools used by physicians.

This preliminary study demonstrates the superiority of live three-dimensional transthoracic echocardiography (3D TTE) over two-dimensional (2D) TTE in the assessment of left atrial (LA) tumors in four patients studied by us (three myxomas, one hemangioma, all subsequently pathologically proven). Because of the unique ability of live 3D TTE to systematically section and view the contents of an intracardiac mass, LA myxomas in the three patients studied could be more confidently diagnosed by noting isolated echolucent areas consistent with hemorrhage/necrosis in the tumor mass. On the other hand, a definite echolucent area was found by 2D TTE in only two of the three patients with myxoma. In the fourth patient with a hemangioma, live 3D TTE showed much more extensive and closely packed echolucencies with little solid tissue as compared to a myxoma consistent with a highly vascularized tumor. In contrast, 2D TTE demonstrated only two isolated echolucencies in the tumor suggesting an erroneous diagnosis of myxoma.

Київська клінічна лікарня на залізничному транспорті № 2 Пухлини серця є рідкісною патологію з поліморфною клінічною картиною і складністю прижиттєвої діагностики. Усі новоутворення серця поділяють на первинні доброякісні (75% від усіх первинних пухлин) і злоякісні (25%), а також вторинні злоякісні пухлини. Більшість випадків серед первинних пухлин серця становлять міксоми, які частіше локалізуються у лівому передсерді (у 75% випадків). Окрім міксоми серця виділяють групу неміксоматозних доброякісних пухлин серця: рабдоміоми, фіброми, ліпоми, лейоміоми, ангіоми, невриноми та інші. Первинні злоякісні новоутворення-це загалом саркоми. вторинні (метастатичні) новоутворення зустрічаються у 13-40 разів частіше, ніж первинні. Клінічні прояви пухлин серця залежать від порушення притоку або відтоку з уражених камер серця, наявності емболічного синдрому, загальної реакції організму на пухлину. Ехокардіографія є основним методом дослідження для початкової діагностики новоутворень серця, що дозволяє визначити розмір пухлини, локалізацію, місце прикріплення, рухливість, співвідношення з клапанним апаратом і характер гемодинаміки. Із сучасних методів дослідження використовують магнітнорезонансну і комп'ютерну томографію. Лікування доброякісних пухлин серця хірургічне. При лікуванні злоякісних новоутворень використовують три методи: хірургічний, променевий і хіміотерапію. У статті представлений клінічний випадок пацієнтки з пухлиною лівого передсердя і перикарда без обструкції кровотоку. Ключові слова: доброякісні пухлини серця, міксома, злоякісні пухлини серця, клінічний випадок.
