Successful reperfusion in relation to the number of passes: comparing outcomes of first pass expanded Treatment In Cerebral Ischemia (eTICI) 2B with multiple-pass eTICI 3

Abstract: BackgroundHigher expanded Treatment In Cerebral Ischemia (eTICI) reperfusion scores after endovascular treatment (EVT) are associated with better outcomes. However, the influence of the number of passes on this association is unclear. We aimed to compare outcomes of single-pass good reperfusion (eTICI 2B) with multiple-pass excellent/complete reperfusion (eTICI 2C/3) in daily clinical practice.MethodsWe compared outcomes of patients in the MR CLEAN Registry with good reperfusion (eTICI 2B) in a single pass to … Show more

“…For FPR, P-to-R time might be more critical than the number of passes of the thrombectomy device. First, in comparison with recanalization by multiple passes of the thrombectomy device, FPR always shows a shorter P-to-R time [9,11,[17][18][19][20][21][22][23]. It means that the P-to-R time could be a significant factor for better clinical outcome even in FPR.…”

“…However, such adverse events were not observed in clinical studies. Arterial injury including dissection is not significantly different between single and multiple passes of the thrombectomy device [17,21]. Considering the fact that endovascular complications by multiple passes of the thrombectomy device are not common, the harmfulness of multiple passes might not be so critical in endovascular treatment outcomes.…”

“…10 However, several studies have reported improved clinical outcome in patients with complete recanalization (TICI 3) versus those with a final score of TICI 2b. [10][11][12][13][14][15] We therefore evaluated different degrees of recanalization and compared the average treatment effects of complete recanalization TICI 3 with TICI 2b on long-term functional outcome.…”

“…Conventionally, the procedural success of MT has been defined by a final Thrombolysis In Cerebral Infarction (TICI) score of better or equal to TICI 2b 10. However, several studies have reported improved clinical outcome in patients with complete recanalization (TICI 3) versus those with a final score of TICI 2b 10–15. We therefore evaluated different degrees of recanalization and compared the average treatment effects of complete recanalization TICI 3 with TICI 2b on long-term functional outcome.…”

“…The conceptual relevance of the first-pass achievement of mTICI grade 2c or 3 has been demonstrated in several clinical studies. Briefly, first-pass recanalization to mTICI grade 2c or 3 was significantly associated with superior clinical outcomes [ 6 11 12 13 14 15 16 17 18 19 ].…”
