Successful medical treatment of 15 dogs with pyothorax

Abstract: In dogs that do not have evidence of pulmonary masses or consolidations and no evidence of granular pleural effusion, medical therapy may be curative even in chronic cases of pyothorax with pleural adhesions.

“…This breed distribution is similar to that reported in studies of dogs with pyothorax in which young working breeds also featured prominently (Demetriou and others 2002, Rooney and Monnet 2002, Johnson and Martin 2007, Boothe and others 2010). All patients were found to have pleural changes on diagnostic imaging similar to those described in a recent study of pyothorax in dogs (Swinbourne and others 2011).…”

“…Mediastinitis and mediastinal abscessation have been most commonly reported as a complication of pyothorax (Rooney and Monnet 2002, Schultz and Zwingenberger 2008, Franklin and others 2011, Brissot and others 2012), the underlying cause for which is often suspected to be inhaled or ingested foreign material (Koutinas and others 2003, Brissot and others 2012). Even if there is no single breed predilection, hunting and working-type dogs are reported to be over-represented (Demetriou and others 2002, Rooney and Monnet 2002, Scott and Macintire 2003b, Johnson and Martin 2007). Surgical management has been recommended for the treatment of selected patients with pyothorax (Demetriou and others 2002, Rooney and Monnet 2002, Boothe and others 2010).…”

“…The presence of an intrathoracic foreign body is an uncommon finding in pyothorax (Demetriou and others 2002). However, surgery (thoracotomy) is warranted in cases of large inhaled foreign bodies and/or mediastinal or pulmonary lesions (Rooney and Monnet 2002, Scott and Macintire 2003b, Johnson and Martin 2007). Potential disadvantages associated with intercostal thoracotomy and sternotomy include wound complications, long recovery and postoperative pain (Ringwald and Birchard 1989, Pascoe and Dyson 1993, Burton and White 1996, Walsh and others 1999, Mellanby and others 2002, Dunning 2003, Orton 2003, Rooney and others 2004, Moores and others 2007).…”

“…Thoracostomy drain (TD) placement is indicated for treatment of pleural disease and for management following thoracic surgery (Tillson 1997a, Younes and others 2002, Johnson and Martin 2007. The decision of when to remove TDs is subjective, and there is wide variation reported in the human literature (Davis and others 1994, Martino and others 1999, Palesty and others 2000.…”
