Successful management of refractory pleural effusion due to systemic immunoglobulin light chain amyloidosis by vincristine adriamycin dexamethasone chemotherapy: a case report

Abstract: IntroductionRefractory pleural effusion in systemic immunoglobulin light chain amyloidosis without cardiac decompensation is rarely reported and has a poor prognosis in general (a median survival of 1.6 months). Moreover, the optimum treatment for this condition is still undecided. This is the first report on the successful use of vincristine, adriamycin and dexamethasone chemotherapy for refractory pleural effusion due to systemic immunoglobulin light chain amyloidosis without cardiac decompensation.Case pres… Show more

“…Response to treatment was defined as the total or partial control of PE or its symptoms. Only 31 (12.4%) patients showed good response to treatment (6 with the first treatment administered,[ 15 20 32 44 54 83 ] 14 with the second,[ 8 11 38 43 46 58 59 60 77 79 80 84 86 96 ] 6 with the third[ 10 47 51 63 78 ] and 5 with the fourth treatment administered[ 28 40 48 66 67 ]). “…There have been two case reports that have explored other chemotherapeutic regimens that have had good results for systemic amyloidosis patients with pleural effusions. In one study, a patient who had previously been receiving intermittent melphalan and prednisone therapy for 7 years was treated with vincristine, adriamycin, and dexamethasone (VAD) therapy with prevention of recurrence of his pleural effusions for the six months that he was on treatment, with return of his pleural effusions only after cessation of therapy [ 18 ]. Ultimately, this patient underwent chemical pleurodesis and continued melphalan and prednisone therapy with no increase in his pleural effusions at four-year follow-up.…”
