Successful intensive chemotherapy followed by autologous hematopoietic cell transplantation in a patient with acute myeloid leukemia and hepatosplenic candidiasis: case report and review of literature

Abstract: Hepatosplenic candidiasis (HSC) in patients with acute leukemia poses management challenges because the therapeutic limitations of the present antifungal armamentarium may adversely impact on treatment outcomes of the underlying leukemia. We report a patient with acute myeloid leukemia who developed HSC during post-remission consolidation chemotherapy and was treated with a prolonged course of caspofungin followed by fluconazole. The stabilization of infection permitted further chemotherapy and autologous hema… Show more

“…Our patient was successfully treated for CDC with caspofungin, but our greatest challenge was continuing cytotoxic agents simultaneously with antifungal therapy to achieve both remission of ALL and control of infection. The literature provides that successful treatment of CDC in patients with ALL does not need to interrupt chemotherapy cycles [ 12 , 15 ]. The reported patient also safely continued both anticancer and antifungal treatment maintaining leukemia and candidiasis remission.…”

“…CDC is usually no acute life‐threatening condition but may require systemic antifungal therapy for months. After stabilisation of signs and symptoms, CDC is not a contraindication for the continuation of chemotherapy or haematopoietic stem cell transplantation or patients even stay in remission with the leukaemia after antifungal therapy 161‐163 …”
