Successful mucosal administration and delivery of drugs still pose a great challenge. However, the possibility to deliver not only small drug molecules but also macromolecular drugs and nanoparticles via mucosal surfaces represents a great opportunity. Rapid onset of drug action, avoidance of first-pass metabolism, and high immunocompetence of mucosa are some of the important features for mucosal drug and vaccine delivery. The use of mucoadhesive drug delivery systems, systems with fast dissolving properties, and nanomaterials with mucus penetration properties are examples of successful strategies to achieve effective mucosal drug and vaccine delivery. Non-keratinized mucosa of the oral cavity, the nasal and vaginal mucosa represent favorable sites of drug administration. Polymer nanofibers have attracted much attention because of remarkable characteristics such as a large surface area to volume ratio and high porosity. Nanofibers have been extensively used for different biomedical applications including wound dressing, tissue engineering, and drug delivery. Among their fabrication methods, the introduction of electrospinning technique was an important step toward achieving the goal of large scale industrial production of nanofiber-based drug delivery systems used in mucosal applications. This chapter provides an overview on all aspects of mucosal drug and vaccine delivery using nanofibers.

Vaccination is regarded as one of the greatest public health achievements and one of the most cost-effective interventions to prevent childhood major illnesses and mortality. However, despite being recognized as one of the most successful public health measures, vaccination is perceived as unsafe and unnecessary by a growing number of individuals that lead to vaccine hesitancy. This study aimed to determine public level of knowledge, attitudes and hesitancy towards child vaccination in Cyberjaya, Selangor. This study also aimed to measure the association between level of knowledge and attitudes with different respondents’ demographic data. Another aim of this study is to measure correlation between knowledge and attitudes with hesitancy towards child vaccination. A guided self-administered questionnaire was used to assess the knowledge, attitudes and hesitancy of the respondents. This cross-sectional study involved 300 respondents identified from the public. Majority of the respondents were females (65.0%), age between 18-29 years (74.7%), Malay (82.3%), Muslim (84.0%), with tertiary education level (78.7%), students (50.0%) and unmarried (70.7%). This study found that the respondents had an overall poor knowledge towards child vaccination (mean knowledge score = 51.3 ± 20.1).

Immunization is one of the most cost-effective and successful public health applications. The results of immunization are difficult to see as the incidence of disease occurrence is low while adverse effects following the immunization are noticeable, particularly if the vaccine was given to apparently healthy person. High safety expectations of population regarding the vaccines so they are more prone to hesitancy regarding presence of even small risk of adverse events which may lead to loss of public trust to the vaccination programs. Vaccine safety monitoring is needed as they now are administered to the general population and also available to special categories such as pregnant women and patients with different diseases whom not subjected to clinical trials as well as incorrect administration rout and presence of rare or delayed onset adverse events make the presence of surveillance system necessary. The aim of the current study was to measure the distribution, percentage, and frequency of adverse reactions related to vaccines administration in Iraq and to assess the causality, severity, seriousness of these adverse reactions. This study is a retrospective descriptive study for surveillance of vaccine safety conducted using Iraqi pharmacovigilance center database from 2014 till the end of 2018 . 2116 Adverse events were included and outcomes , severity , seriousness , and causality of adverse events were assessed. Majority of adverse events following immunization cases (90.97%) were mild, non serious (94.47%) and recovered (94.23%). Most reports were for general disorders and administration site conditions and the majority were for elevated body temperature and injection site reactions .
