Substitution treatment of injecting opioid users for prevention of HIV infection

Abstract: Oral substitution treatment for opioid-dependent injecting drug users is associated with statistically significant reductions in illicit opioid use, injecting use and sharing of injecting equipment. It is also associated with reductions in the proportion of injecting drug users reporting multiple sex partners or exchanges of sex for drugs or money, but has little effect on condom use. It appears that the reductions in risk behaviours related to drug use do translate into reductions in cases of HIV infection. T… Show more

“…Medications are a proven component of comprehensive substance abuse treatment plans that reduce drug use and provide an opportunity for improvement in health and social functioning for individuals with opioid dependence ( Fig. 7 ; Gowing et al 2004 ; Johnson and McCaugh, 2000 ; NIDA, 2000a ). Two recent TIPs from SAMHSA, TIP 40 and TIP 43, provide the best practices guidelines for the use of either methadone or buprenorphine as part of a comprehensive treatment plan for opioid abuse/dependence ( SAMHSA, 2004 ; SAMHSA, 2005 ).…”

“…MAT is effective in treating heroin dependence thereby reducing associated risks including Hepatitis C, HIV and death (Amato, Davoli, Ferri, Gowing, & Perucci, 2004; Gowing, Farrell, Bornemann, & Ali, 2004). By making clean needles and syringes available, NSP decreases drug-related risk behaviors such as sharing of injection equipment (Strathdee, 2001).…”

“…The most evaluated secondary prevention intervention is the prescribing of non-injected substitute drugs, in particular oral methadone [ 8 ]. There is evidence that this can reduce injection frequency and illicit drug consumption and is also associated with reductions in morbidity and mortality [ 9 - 12 ]. It is uncertain whether and how drug treatment modifies overall duration of drug dependence and injecting.…”
