Subperiosteal resection of fibular aneurysmal bone cyst

Abstract: Aneurysmal bone cyst (ABC) of the fibula poses a challenge as to the most appropriate treatment particularly in children and adolescents. A prospective study was conducted to evaluate the results of subperiosteal resection of fibular ABCs. Eleven patients (six boys and five girls) with an average age of 12.4 years (6-18 years) were treated by subperiosteal resection of ABC of the fibula. The lesion was juxtaphyseal in three patients (two distal and one proximal), metaphyseal in seven and diaphyseal in one. All… Show more

“…Studies pertaining to en bloc excision of ABCs report 95-100 % localized control [16][17][18][19]. Flont et al retrospectively reviewed 26 patients undergoing en bloc excision and reported no recurrences; however, morbidity (postoperative pain, limb length discrepancies, muscle weakness, and decreased ranges of motion) was increased with en bloc procedures compared to intralesional procedures [20].…”

“…To date, the main treatment choice for bular tumor is segmental or subperiosteal resection, in case of local recurrence at surgical site [17][18][19]. Given that preoperative digital radiograph indicated that the vascular mass on bula was solitary, and part of both cortex and cancellous bula were not involved, intralesional curettage was selected as the surgical intervention in this case for achieving the maximum retention of healthy bony structure.…”

“…On the other hand, wide en-bloc excision is an excellent option with the lowest rates of recurrence (95%-100% localized control)[ 14 ]. Moreover, it leads to reduce risk of future malignant transformations.…”
