Aims: The decision on the appropriate type of anesthesia for pelvic floor repair depends on a variety of factors including patients' age, performance status, comorbidities, cost-effectiveness and personal preferences. We aim to review the literature on urogynecological procedures performed under local anesthesia (LA).Methods: A systematic search of four electronic databases was conducted for articles published up to May 2020. Studies reporting outcomes of women who underwent pelvic floor reconstructive surgery under LA with or without sedation, were considered eligible.Results: Nineteen studies (14 noncomparative and 5 comparative), including 1626 cases of urogynecological procedures under LA were recruited. Metaanalysis revealed significantly lower mean pain scores in LA group compared to general-regional anesthesia one (GA/RA) at both 4-6 h and 8-18 h postoperatively (160 patients; mean difference [MD], −1.70; 95% confidence interval [CI]: −3.12, −0.28; p = 0.02 and 160 patients; MD, −0.72; 95% CI: −1.17, 0.27; p = 0.002, respectively). Pain scores at >24 h did not differ among the two groups (160 patients; MD, −0.28; 95% CI: −0.60-0.05; p = 0.10). Intra-and postoperatively morphine use was not different among patients who received LA and GA during prolapse surgery while nausea rates were significantly lower in LA group compared to RA group 8 h postoperatively. Conclusions: LA with or without sedation represents a safe and efficient alternative anesthetic technique for urogynecological procedures with improved pain scores in up to 18 h postoperatively especially in patients who underwent surgery for SUI. LA is feasible and could be offered to patients undergoing pelvic floor surgery allowing a prompt postoperative recovery.

Hypothesis/aims of study. An integrated evaluation of the effectiveness of pelvic organ prolapse (POP) treatment in women requires a consistent interpretation of the criteria of treatment satisfaction. The aim of this study was to specify surgical treatment satisfaction in patients with POP. Study design, materials, and methods. Surgical treatment was performed in 486 patients with POP stages II–IV. In 12 months, a patient survey (using a 1-5-point scale) evaluated the level of treatment satisfaction, reasons for lowered self-reported satisfaction, and respondents’ ambivalence regarding treatment results. Results. One-year post-surgical treatment of POP stages 0–I are observed in 80.9% of patients. Full satisfaction (5 points) was self-reported by 61.1% of patients, 28.6% of patients indicated partial satisfaction (3–4 points), and 10.3% of patients indicated treatment dissatisfaction (1–2 points). Post-surgery stage of POP is weakly correlated with self-reported treatment satisfaction (r = –0.39; p < 0.0001). Most common reasons for lower self-reported satisfaction were the emergence of new issues, fear of pathology recurrence, urinary incontinence, and pain. Five patients (1.0%) indicated that their condition “became much worse” after the surgical treatment. Conclusion. The treatment satisfaction range is variable. It is necessary to consider a variety of clinical, deontological, and psychological factors in the analysis of surgical POP treatment satisfaction spectrum.

Resultados: A idade média foi 60 anos (DP=10,3); 76% apresentaram nível educacional básico ou médio, com companheiro (61%), com atividade sexual (40%), com menopausa (74,5%); 2,5 procedimentos cirúrgicos para correção do POP em média. A QVRS melhorou após o tratamento cirúrgico, em ambos os seguimentos (p<0,001), e em todas as dimensões. A imagem corporal apresentou diferença significativa (p<0,05) em todos os momentos de avaliação: para cada ponto de piora da imagen corporal, menor mudança na QVRS após a cirurgia. Os sintomas menopáusicos impactaram em menores mudanças na QVRS nas dimensões PGS e E, em todos os tempos, embora a QVRS nas dimensões IP, LF, LS e SE tenha apresentado leve porém significativa melhora após a cirurgia (p<0,05). As mulheres com doença neurológica mostraram menores mudanças na QVRS no pós-operatório comparativamente àquelas sem essa doença nas dimensões LF, E e SE (p<0,05). Outras variáveis preditoras de QVRS foram a atividade sexual, nível educacional, idade e diabetes mellitus. Obteve-se um efeito de grande magnitude em todas as dimensões e estabeleceu-se MID entre-1,19 e-15 pontos para todas as dimensões de PQOL. Conclusão: A QVRS melhorou após o tratamento cirúrgico, em todas as dimensões, e as variáveis preditoras mais importantes foram a imagen corporal, os sintomas menopáusicos e as doenças neurológicas. O estudo contribui para corroborar a necessidade de avaliação integral das mulheres no pré-operatório visando a conhecer as tendências de sua QVRS posteriormente atuando nos fatores modificáveis que podem influir negativamente em sua recuperação pós-operatória. Abstract: At 5-year follow-up 78 % was relieved from their bulge symptoms by an operation using local anesthesia. Eleven percent of the women had been reoperated.
