Subepicardial aneurysm detected during diagnosis of acute cardiac failure

Abstract: A 55-year-old female developed dyspnea at night and visited our hospital. A diagnosis of acute cardiac failure was made and the patient was immediately admitted. A false ventricular aneurysm with blood flow was found in the lateral wall of the left ventricle. Past history of myocardial infarction was not clear. Repeated cardiac failure may have resulted from decreased cardiac output by increased blood flow in the aneurysm due to its expansion. After patch closure was performed, the symptoms improved.

“…Previous studies have emphasized the lack of myocardial fi bers and epicardial coronary vessels at the site of a left ventricular aneurysm, thus suggesting a possible role for them in the development of the aneurysm. 2,3 Similarly, the left ventricular apical region where the aneurysm had developed was avascular.…”

“…In addition, it may sometimes be challenging to determine whether the aneurysm is a true aneurysm, a pseudoaneurysm, or a diverticulum. 2,4,5 Furthermore, mitral valve insufficiency may also accompany a left ventricular aneurysm. Appropriate mitral valve replacement can be performed using a transventricular approach.…”
