Subcutaneous epidermal inclusion cysts: Ultrasound (US) and MR imaging findings

Abstract: On US, subcutaneous epidermal inclusion cysts are usually well-circumscribed, oval-shaped, mildly echogenic masses with occasional linear anechoic and/or echogenic reflections, increased through-transmission, hypoechoic rim and no Doppler flow. On MR, an intermediate to high T2 signal mass with occasional low signal debris and no central enhancement can strengthen the diagnosis.

“…On MRI, it is well encapsulated and exhibits hypointense signal on T1-weighted image. On T2-weighted image, it is hyperintense to intermediate in signal intensity with multiple hypointense internal debris, which are seen in 43% of cases 4. On T1-weighted fat-saturated images, the cyst content remains hypointense but the debris may appear hyperintense, as seen in the present case.…”

“…42,43 Epidermal cyst, most commonly an incidental finding, does not enhance on both CT and MR, unless infected, whereas DFSP shows variable enhancement on CT and MR imaging. 44,45 Peripheral nerve sheath tumors are usually close to a major nerve, with certain signs such as split fat or fascicular or target sign, which are absent in DFSP. 45,46 Plexiform neurofibromas tend to be asymmetric and diffuse, whereas DFSP most commonly is nodular.…”

“…Melanomas can have associated lymphadenopathy and osseous destruction not usually found in DFSP. 44,51,52 Mycosis fungoides is the most common type of cutaneous T-cell lymphoma. CT findings include skin thickening, edema, soft tissue infiltration, lymphadenopathy, and systemic manifestations, not typically found in DFSP.…”

“…An intermediate to high T2 signal mass with occasional low signal debris associated with no central enhancement on Magnetic Resonance Imaging (MRI) has been reported to be helpful in strengthening the diagnosis. 12 Initial misdiagnosis of epidermal cyst can be due to its ill-defined borders and heterogenous echogenicity. 6,13 Excision of such lesions should be offered to relieve symptoms, prevent complications and in some cases, for cosmetic reasons.…”
