Subclinical impairment of arterial mechanics in systemic lupus erythematosus identified by arterial waveform analysis

Abstract: Structural and functional changes in wall and endothelial components of arterial blood vessels underlie the accelerated vascular disease progression in systemic lupus erythematosus (SLE). Using pulse contour analysis we sought to determine if subclinical vascular abnormalities could be identified in a well-characterised cohort of patients with SLE who had no increase in traditional cardiovascular risk factors. Radial artery pressure waveforms were obtained by applanation tonometry and pressure envelopes were a… Show more

“…Increased PWV, a marker of adverse cardiovascular prognosis in various adult populations (5), has been demonstrated in adult CID patients (11, 32) and children at high cardiovascular risk (7, 33). LAEI and SAEI are relatively novel markers of arterial compliance that have been associated with cardiovascular risk factors (34, 35) and CID in adults (36), but their value in cardiovascular prognosis is not known. No previous studies assessed LAEI and SAEI in patients with JIA.…”

“…8,9 Our group and others have shown that an alteration in arterial mechanics occurs in SLE and this finding may contribute to the development of atherosclerosis in these patients. 10,11 Furthermore, we have shown the potential importance of microcirculatory abnormalities in SLE with patients exhibiting impaired brachial artery flow-mediated dilation and altered microvascular responses to a period of forearm ischaemia. 12 Structural and functional changes in different vascular territories are known to provide predictive information in relation to arterial disease progression and future micro-and macro-vascular complications.…”
