Subcarinal ventilation-assisted Y-shaped stent insertion under local anesthesia for patients with complex tracheobronchial stenosis: initial clinical experience

Abstract: PURPOSEWe aimed to report our preliminary results of subcarinal ventilation-assisted Y-shaped stent insertion under local anesthesia for patients with complex lower tracheal-carinal-main bronchial complex stenosis. MATERIALS AND METHODSSeven consecutive patients with lower tracheal-carinal-main bronchial complex stenosis underwent Y-shaped stent insertion under local anesthesia. During the procedure, subcarinal ventilation was performed using a 4 F angiographic catheter, and stent insertion was performed under… Show more

“…Although surgical resection is still the best treatment for NSCLC, most patients were diagnosed at the advanced stage and were not candidates for surgical treatment [10][11][12]. The first pass effect is the mechanism of TAI.…”

“…Many retrospective studies have considered the survival effects of airway stent applications. Survival has been reported to be 109‐181 days with a Y‐type silicon stent and 116‐217 days with Y‐shaped SEMS . In our study, survival in the silicon and SEMS groups were 164.51 ± 38.83 days and 254.45 ± 103.32 days, respectively ( P > .05).…”

“…Two case series published in 2006 and 2008 have demonstrated good feasibility and high success rates. Overall, to date, 12 case series (including 370 patients) describing the use of tracheobronchial SEMS have been published, with similar success rates, efficacy and median survival as the silicone stents 6,19,28–37 . A review of the insertion technique shows that despite the Y SEMS advantage, not requiring the use of a rigid bronchoscopy, its limitation is a wide delivery system that requires a large caliber deployment conductor.…”
