IntroductionTimely and accurate diagnosis of the earliest manifestations of Alzheimer’s disease (AD) is critically important. Cognitive challenge tests such as the Loewenstein Acevedo Scales for Semantic Interference and Learning (LASSI-L) have shown favorable diagnostic properties in a number of previous investigations using amyloid or FDG PET. However, no studies have examined LASSI-L performance against cerebrospinal fluid biomarkers of AD, which can be affected before the distribution of fibrillar amyloid and other changes that can be observed in brain neuroimaging. Therefore, we aimed to evaluate the relationship between LASSI-L scores and CSF biomarkers and the capacity of the cognitive challenge test to detect the presence of amyloid and tau deposition in patients with subjective cognitive decline and amnestic mild cognitive impairment (MCI).MethodsOne hundred and seventy-nine patients consulting for memory loss without functional impairment were enrolled. Patients were examined using comprehensive neuropsychological assessment, the LASSI-L, and cerebrospinal fluid (CSF) biomarkers (Aβ1-42/Aβ1-40 and ptau181). Means comparisons, correlations, effect sizes, and ROC curves were calculated.ResultsLASSI-L scores were significantly associated with CSF biomarkers Aβ1-42/Aβ1-40 in patients diagnosed with MCI and subjective cognitive decline, especially those scores evaluating the capacity to recover from proactive semantic interference effects and delayed recall. A logistic regression model for the entire sample including LASSI-L and age showed an accuracy of 0.749 and an area under the curve of 0.785 to detect abnormal amyloid deposition.ConclusionOur study supports the biological validity of the LASSI-L and its semantic interference paradigm in the context of the early stages of AD. These findings emphasize the utility and the convenience of including sensitive cognitive challenge tests in the assessment of patients with suspicion of early stages of AD.

Background: Some studies have suggested an association between the month of birth and risk of multiple sclerosis (MS), related to environmental factors, mainly sun exposure and maternal vitamin D levels. Few studies have been conducted in Southern Europe countries. Madrid has a continental climate with considerable variation of sun hours between winter and summer, so it may be relevant to study this relationship. Methods: MS patients, born between 1932 and 2003, 1,335 of them from our database between 2004 and 2015, were analysed. The weighted average number of births per month in Madrid from 1996 and 2012 (n = 1,098,774) was considered the control population. The month and season of birth were analysed using chi-square, Hewitt's and Roberson's seasonality tests. Results: Birth rate increased in June, July, and September, and decreased in November, January, and February. Births were 29% more frequent in summer (July-September) than in winter (January-March), with a ratio of 0.79. Hewitt's test for seasonality gave a rank sum of 53 between May and October (p = 0.12). Rogerson's variation was applied to 3-, 4-, and 5-month periods. Substantial differences were noted in the 5-month periods (k = 5), although the largest rank sum (June-October) was not significant (p = 0.09). Conclusion: Our analysis seemed to suggest that pre-natal sun exposure may have an influence on the incidence of MS, most likely in combination with other environmental or genetic factors.

Amyloid and FDG-PET study of logopenic primary progressive aphasia: evidence for the existence of two subtypes

The logopenic variant of primary progressive aphasia (lvPPA) has been associated with Alzheimer disease, although this relationship is still subject to debate. The purpose of this study is to determine the frequency of amyloid biomarkers in patients with lvPPA, and record any potential clinical or topographic differences between patients with and without amyloid deposits. We conducted cognitive examination and positron-emission tomography studies with fluorodeoxyglucose ((18)F) and florbetapir ((18)F) in a cohort of 16 patients diagnosed with lvPPA. We evaluated the prevalence of amyloid deposits as well as any clinical and metabolic differences between the groups with and without significant presence of amyloid deposits. Eleven patients (69 %) were considered amyloid-positive. The amyloid-positive group displayed less metabolic activity in the left temporoparietal region than the control group, while the amyloid-negative group showed lower metabolism in the left temporoparietal region extending to the anterior temporal and basal frontal regions. The percentage of change in patients with clinical and FDG-PET follow-up did not differ between the amyloid-positive and amyloid-negative subgroups. The frequency of amyloid-positive cases confirms that lvPPA is frequently associated with Alzheimer disease. Amyloid-negative patients show a different cerebral metabolic pattern. These findings show the relevance of using amyloid PET to study lvPPA, and also suggest that the logopenic variant may not be specific to Alzheimer disease in certain cases.
