Studies on the Mechanism of Erythrocytosis Associated with a Uterine Fibromyoma

Abstract: Summary. Erythrocytosis has occasionally been associated with uterine fibromyoma and attributed to an inappropriate elaboration of erythropoietin (Ep) by the tumour. Whether Ep per se, its precursor substrate, or activator analagous to the renal erythropoietic factor (REF) is produced, has not been defined. In a 54‐yr‐old woman with erythrocytosis and uterine fibromyoma, plasma, urine and tumour extracts were examined to clarify these possible mechanisms. Preoperatively, elevated serum and urinary Ep were pre… Show more

“…As the kidney is probably the major site for erythropoietin production this may not be ectopic. Since then, there are three well-documented reports of erythrocytosis with uterine fibromyomata, where removal of the tumour has cured the erythrocytosis and satisfactory tumour erythropoietin measurements have been made (Hertko, 1968 ;Wrigley et al, 1971;Ossias et al, 1973). Wrigley (1970) demonstrated high erythropoietin concentrations in the fluid taken from a cystic cerebellar haemangioblastoma associated with erythrocytosis.…”

“…It is generally accepted that the increased production of erythrocytes in all cases of secondary polycythemias is associated with increased Ep formation (3). Such an association can not be demonstrated in polycythemia vera (PV).…”

“…Hertko (1963) and several other authors have shown marked erythropoietin activity in uterine fibromyomas, whereas others failed to demonstrate such activity (Fried et al 1968; Ossias et al 1973; Payne et al 1969). The fact that erythropoietin was occasionally not measurable in the plasma in several cases could be related to the lack of sensitivity of the assay, insufficient to detect normal levels of erythropoietin, and probably insensitive also to moderate increases of the hormone.…”
