Studies on the mechanism of sinus node dysfunction in the sick sinus syndrome.

Abstract: The intrinsic heart rate (IHR) was determined in 17 patients with symptomatic sinus bradycardia by administering atropine 0.04 mg/kg and propranolol 0.2 mg/kg, i.v. In this way, sick sinus (SSS) patients with intrinsic sinus node (SN) dysfunction could be distinguished from those patients with disturbed autonomic regulation of SN function. Sick sinus syndrome patients with normal corrected sinus node recovery time (SNRTC), adjusted for the magnitude and direction of autonomic chronotrophy, consistently had nor… Show more

“…The sensitivity of IHRp is very low for diagnosing sinus node dysfunction. Initially the same dosages were used for distinguishing between intrinsic and extrinsic sinus node dysfunction when assessing sinus node recovery time [164,165] , and later modified to 75% by Tonkin et al [166] . From a diagnostic point of view it seems sufficient to use propranolol 0·1 mg .…”

“…RECENT interest in sinus node function in man has led to an increasing awareness of the importance of vagal tone as a factor that modulates the degree of disturbance of nodal function (Dighton, 1974(Dighton, , 1975Jordan et al, 1978). The arrhythmias resulting from such a disturbance of nodal function include sinus bradycardia, sinus pause or arrest, and sinoatrial exit block.…”

“…Patients su!ering from arrhythmia originating in the SN area are said to have the &&sick sinus syndrome'' (Rubenstein et al, 1972;Ferrer, 1973;Kaplan et al, 1973;Jordan et al, 1978;Delon et al, 1992). In this syndrome, sinus rhythm is most often slower than normal, but there may be paroxysmal attacks of tachycardia or atrial "brillation.…”
