Endometrial glandular cells and mononuclear phagocytic cells dominated the samples of undiluted uterine fluid obtained at different stages of the menstrual cycle whereas granulocytes were present in much lower numbers. Mast cells were occasionally found. The number of each cell type was significantly decreased in the luteal phase as compared to the midcycle and premenstrual phases. Phagocytic activity of the mononuclear cells was also significantly decreased in the luteal phase. Low numbers of inflammatory cells and low phagocytic activity during the luteal phase may be important for survival of the blastocyst in the event of conception.

The timing of the procedure is important as sperm migration does not usually occur in the early follicular and luteal phases of the menstrual cycle (Croxatto et al, 1973). Four patients had to be excluded because evidence obtained subsequently showed that they had been in the luteal phase at the time of laparoscopy.…”

Summary Sperm recovery from the peritoneal fluid was attempted in a group of infertile women in whom it was anticipated that all routine investigations would be normal (commonly referred to as ‘normal infertiles’). Forty‐four couples were recruited into the study but in retrospect only 24 met the study criteria of normal semen, normal pelvis and appropriate timing. In 15 of these 24 patients, spermatozoa were covered from the peritoneal fluid, and so far seven have become pregnant. In the nine patients where spermatozoa were not recovered, none have so far become pregnant. There was no correlation between sperm recovery and the quality of the post‐insemination cervical mucus examination. This test allows more accurate assessment of the ability of spermatozoa to reach the site of fertilization.

A review is given on sperm migration to and sperm survival within the human Fallopian tube. Sperm migration from the external os can be very fast. The survival time of spermatozoa in the oviduct has been demonstrated to be 85 h. Spermatozoa normally enter the abdominal cavity through the open fimbriated end. Laterally closed oviducts retain spermatozoa resulting in a larger number of spermatozoa than in the normal oviduct, where the number of sperm at the site of fertilization is very low. The morphology of spermatozoa reaching the ampulla of the oviduct is mostly normal, which seems to be based on the correlation between normal morphology and good motility. Spermatozoa within the abdominal cavity do not cause antibody formation of any importance for the fertility of the woman.
