Introduction: The quality of life of patients with heart failure is influenced by several factors, one of which is spiritual factors. Tri Hita Karana (THK) is one of the concepts of the life order of the Balinese people, which basically teaches about maintaining harmony between humans and God (prahyangan), humans and humans (pawongan), humans with the environment (palemahan). This study aimed to determine the relationship between THK implementation on the quality of life of heart failure patients at Regional Hospital of Mangusada. Methods: A cross-sectional design study was used. In total 41 heart failure patients who visited Mangusada Hospital between April to May 2020, selected using a purposive sampling technique. The research instrument used was the attitude and behavior implementation of THK and the Minnesota Living with Heart Failure Questionnaire (MLHFQ). The Spearman rank test was used in this study. Results: There were a significant correlation between Tri Hita Karana's Implementation and Life Quality of Heart Failure Patients in Mangusada Regional Hospital with p-value (0.001), where p < α (0.05). Conclusion: The better the implementation of THK, the better the quality of life for patients with heart failure. The results of this study were expected to be used in providing education to patients by implementing THK to improve the quality of life for patients with heart failure.

Coronary heart disease is a disease caused by flak that accumulates in the arteries which reduces the supply of oxygen throughout the body, especially the heart muscle. A good quality of life in patients with coronary heart disease is needed to maintain that patients are able to get the best health status, optimally and for as long as possible. The study aims to find out the Factors Affecting the Quality of Life of Patients with Coronary Heart Disease at the Sikumana Health Center. A quantitative descriptive research method using a cross sectional approach. The results of the 3 variables showed that depression had a significant relationship to quality of life p-value = 0.028, coping had no effect on the quality of life of heart sufferers p-value = 0.078 and family support no effect on the quality of life of people with heart disease with p-value = 0.101, multiple multivariate analysis with the backward LR method, it can be seen that the R Square value is 0.335 meaning that coping and family support variables explain the quality of life of heart patients by 33.5% while the rest is explained by other variables not examined in this study. based on the beta coefficient, the independent variable that has the greatest beta coefficient value is family support (coef. beta = -1.867). It can be concluded that the independent variable that has the most influence on the quality of life of heart sufferers is family support

Congestive Heart Failure (CHF) adalah kondisi dimana terjadinya kegagalan pada jantung kanan dan kiri. CHF ditandai dengan adanya kelebihan cairan atau perfusi jaringan salah satunya adalah efusi pleura. Efusi pleura diakibatkan karena adanya peningkatan tekanan vena sistemik dan tekanan kapiler paru, hal tersebut membuat aliran oksigen ke jaringan tubuh menjadi berkurang dan menyebabkan tubuh mengalami desaturasi yang ditandai dengan menurunnya tingkat saturasi oksigen dalam tubuh dan timbul sesak sebagai respon tubuh. Pasien dalam penelitian ini berusia 43 tahun dan merupakan ibu rumah tangga. Pasien datang dengan keluhan sesak ditandai dengan frekuensi napas 24x/menit dan penurunan saturasi 94%. Kondisi sesak napas yang tidak teratasi dapat menimbulkan gangguan kenyamanan pada pasien dan mengganggu aktivitas sehari-hari, selain itu pasien mempunyai riwayat hipertensi yang tidak terkontrol yang menjadi salah satu faktor risiko terjadinya CHF. Penelitian ini bertujuan untuk melihat gambaran intervensi manajemen sesak pada pasien CHF dengan efusi pleura. Metode yang digunakan dalam penelitian ini adalah case report melalui proses asuhan keperawatan secara komprehensif untuk manajemen sesak. Adapun intervensi yang dilakukan untuk mengatasi sesak adalah dengan pemantauan status pernapasan, pemberian posisi semifowler dan lateral kanan serta kolaborasi pemberian terapi oksigen. Evaluasi didapatkan frekuensi napas dalam rentang normal dan reguler, terdapat peningkatan saturasi oksigen, tidak terdapat penggunaan otot napas tambahan, pasien mengatakan sesak berkurang dan terdapat penurunan skala sesak dari skala 4 menjadi skala 2. Pada pasien ini, manajemen sesak berupa pemantauan status pernapasan, pemberian posisi semifowler dan lateral kanan serta kolaborasi pemberian oksigen dapat mengurangi sesak pada pasien CHF dengan efusi pleura.
