Student-Run Clinics: A Novel Approach to Integrated Care, Teaching and Recruitment

Abstract: The purpose of this study is to assess the educational impact of incorporating a psychiatry consult service into a medical student-run clinic. In November 2014, a psychiatry consult service was introduced at the Columbia-Harlem Homeless Medical Partnership (CHHMP), a student-run clinic located in Harlem, New York. From September 2015 to January 2016, surveys (Qualtrics) were then distributed to student members of the clinic. Surveys assessed how the psychiatric consult service impacted students' knowledge, ski… Show more

“…40 Another reason for the lack of change in attitudes could be the specific components of the psychiatry clerkship. Recently, a number of contextual clerkship factors, such as interpersonal skills training related to patient management, 41 educational models like student-run clinics to increase medical students' perceived knowledge and comfort with psychiatry 42 , and enthusiastic and positive clinical staff encouraging the students' social integration 43 , have been proposed as influential in furthering students' long-term positive attitude towards psychiatry. Thus, efforts to improve attitudes towards psychiatry may require more than just one two-hour lecture.…”

“…SRCs predominantly target the primary care needs of uninsured patients, with an emphasis on health care maintenance and chronic disease management (Smith et al, 2014 ). Several SRCs have offered mental health screening and care as increasing needs for these services are recognized (Batra et al, 2009 ; Doyle et al, 2012 ; Fischbein et al, 2020 ; Khonsari et al, 2019 ; Knoll et al, 2021 ; Liberman et al, 2011 ; Mann et al, 2019 ; Mishan & Dragatsi, 2017 ; Schweitzer & Rice, 2012 ; Soltani et al, 2015 ; Stuard et al, 2019 ).…”

“…Patients from racial, ethnic, and sexual and gender minorities experience barriers to accessing care in addition to stress related to prejudice and discrimination, which are risk factors for mental health disorders (Bostwick et al, 2014 ; Lee et al, 2016 ; Mays et al, 2007 ; Williams et al, 2003 ). To expand psychiatric services in primary care settings, some clinics have developed a model in which medication management occurs under the direct guidance of a consulting psychiatrist (Batra et al, 2009 ; Mishan & Dragatsi, 2017 ). SRCs have also begun to incorporate dedicated mental health treatment teams, allowing psychiatrists to work directly with medical students to provide medication management services with supportive interventions (Doyle et al, 2012 ; Khonsari et al, 2019 ; Knoll et al, 2021 ; Mann et al, 2019 ; Schweitzer & Rice, 2012 ; Soltani et al, 2015 ).…”

“…Student-led musculoskeletal clinics are typically associated with a university and function similar to that of a private practice, but are run by student physiotherapists under the supervision of fully qualified physiotherapists known as 'clinical educators' (Mishan & Dragatsi, 2017). Student-led clinics often run at a significant cost to their respective university and are, therefore, used to supplement clinical education and provide low-cost health services to the local community.…”

“…Compared to private practice where patient care may be prioritised over student learning, student-led musculoskeletal clinics are designed to maximise student learning opportunities (Meah et al, 2009). As such, students completing clinical education within studentled musculoskeletal clinics may manage lower patient numbers, compared to private practice or hospital outpatient departments; however, this is balanced out by longer appointment times that allow for greater student to clinical educator interaction and reflection (McBride et al, 2018;Mishan & Dragatsi, 2017). Furthermore, the pathways for patient referral also differ between outpatient departments, private practice, and student-led clinics, which not only influence the number of patients managed by students, but also the type of the clinical conditions they manage.…”
