Structural abnormalities of the trigeminal root revealed by diffusion tensor imaging in patients with trigeminal neuralgia caused by neurovascular compression: A prospective, double-blind, controlled study

Abstract: Because diffusion tensor imaging (DTI) is able to assess tissue integrity, we used diffusion to detect abnormalities in trigeminal nerves (TGN) in patients with trigeminal neuralgia (TN) caused by neurovascular compression (NVC). We also studied anatomical TGN parameters (cross-sectional area [CSA] and volume [V]). Using DTI sequencing in a 3-T magnetic resonance imaging (MRI) scanner, we measured the fraction of anisotropy (FA) and the apparent diffusion coefficient (ADC) of TGN in 10 patients selected as can… Show more

“…8,21,27 FA and ADC represent the 2 DTI indices that are most widely used because they reliably indicate abnormalities in white matter tracts, independent of their cause. 3,26,30 The measurement of FA provides robust values for quantifying the degree of directionality of diffusion (i.e., anisotropic diffusion) as a marker of microstructural integrity.…”

“…More recently, two teams used 3-T MRI to measure V and/or CSA. 6,17 According to Fujiwara et al, 6 the mean CSA on the ipsilateralTN in 13 patients (3.9 ± 0.8 mm 2 ) was significantly smaller than that on the contralateralTN (5. ).…”

“…5,6,11,13,17 In 2005, Kress et al 13 measured V in 62 patients (41 with TN, including 6 patients with multiple sclerosis) with one investigator using 1.5-T MRI in 50 normal subjects. In that study, the mean V on the ipsilateralTN was 69.6 mm 3 (95% CI 37.2-110.6) and that on the contralateralTN was 84.5 mm 3 (95% CI 50.3-141.1).…”

“…This neuropathic disorder can be profoundly distressing and may negatively affect wellbeing (7). Compression of the trigeminal nerve root by an artery, or more rarely a vein, is demonstrable in 90-95% of patients, according to findings from a series of patients who underwent microvascular decompression for TN (8). TN often causes depression and has even resulted in suicide in some individuals.…”
