Оценить качество оказания нейрохирургической помощи пациентам при цереброваскулярных болезнях (ЦВБ) в Украине, определить пути ее совершенствования. Материалы и методы. Анализ проведен на основе статистических сведений о лечебной работе нейрохирургических отделений Украины за период 2006-2015 гг. Использованы статистические методы выборки и группировки, исследования динамических рядов. Подсчеты осуществлены с помощью программы Microsoft Office Excel. Результаты. За 10 лет число пациентов с ЦВБ, госпитализированных в нейрохирургические отделения страны, увеличилась на 76% (с 143 на 1 млн. населения-в 2006 г. до 252-в 2015 г.). В 2015 г. на 1 млн. оперированы 119 пациентов по поводу ЦВБ, что в 2,3 раза больше, чем в 2006 г. (51). Доля пациентов с ЦВБ в структуре госпитализированных за 10 лет увеличилась с 7 до 10%, в структуре оперированных-с 6 до 11%. Хирургическая активность с 36%-в 2006 г. увеличилась до 47,4%-в 2015 г. (темп прироста-32%). Отмечено снижение летальности: общей-с 10,5 до 8,9% (темп снижения-15%), послеоперационной-с 16,9 до 8,9% (темп снижения-47%). Лидером по абсолютным показателям предоставления нейрохирургической помощи пациентам с ЦВБ является Киев. В 2015 г. в нейрохирургические отделения столицы госпитализированы 4173 пациентов с ЦВБ (46% всех больных в Украине-9146); оперированы 2177 (50% всех оперированных в стране-4336). В 2015 г. наиболее обеспеченными нейрохирургической помощью при ЦВБ были Ивано-Франковская (17,9%), Днепропетровская (16,2%), Львовская (12,1%) области, в 8 областях показатель обеспеченности был в пределах 5-10%; в 10-меньше 5%. В Сумской области нейрохирургическую помощь пациентам по поводу ЦВБ не оказывали. В целом по Украине обеспеченность нейрохирургической помощью при ЦВБ составила 13,3%. Выводы. За 10 ле т чис ло пациен т ов с ЦВБ, ко т орым оказана специализированная нейрохирургическая помощь, увеличилось более чем вдвое. Отмечено значительное повышение хирургической активности и снижение общей и послеоперационной летальности. Уровень оказания нейрохирургической помощи пациентам по поводу ЦВБ значительно различается в регионах. Отмечен низкий уровень обеспечения потребности пациентов с ЦВБ в нейрохирургической помощи со значительными различиями показателя в регионах Украины. Целесообразно создать 7-9 нейрохирургических межрегиональных «центров совершенства». Неотложными являются подготовка и внедрение государственной программы противодействия ЦВБ. Today, cardiovascular diseases are one of the main causes of disability of the population. Most of the illnesses, including stroke, are accompanied by the appearance immunoglobulin G (IgG) in the blood circulation. According to the literature sources and previous experiments, it is known that IgG made influence on the hemostasis system. Objectives of the investigation are pure enzymes: thrombin, factor X, proenzyme of protein C and prothrombin under the influence of IgG fraction that were separated from the plasma of patients with atherothrombotic and cardioembolic ischemic stroke and patients with other neurological diseases. IgG was separated by affinity chromatography. Disc electrophoresis was used to control antibodies' purity. The main goal of the experiment is to test the potential influence of all fractions of separated IgG on the process of hydrolysis of specific chromogenic substrates by the key factors of the hemostasis system such as thrombin, factor X and protein C. The appearance of IgG in the blood stream during the atherothrombotic and cardioembolic ischemic stroke and other neurological diseases was proved. Concentration of the IgG in plasma of patients with cardioembolic ischemic stroke was significantly higher compared with healthy donors, whereas the IgG in plasma of patients with atherothrombotic ischemic stroke and other neurological disorders was not significantly different. Their potential ability to influence the enzymes such as thrombin, factor X, proenzyme of protein C and prothrombin was shown. It was proved that the antibody fractions of all experimental groups significantly accelerate the process of splitting chromogenic substrate by thrombin but inhibited cleavage of a specific chromogenic substrate by protein C.

Introduction. The significant prevalence of vascular diseases of the brain, the complexity of dysfunc- tion, put the problems of their treatment, rehabilitation and prevention into the focus of attention of modern neurology. A stroke occurs in the United States every 45 seconds. Approximately 750,000 people a year suffer from a stroke in the United States. According to statistics from the Ministry of Health of Ukraine, more than 110,000 primary strokes are recorded annually in the country. No less serious disease of the human central nervous system is Alzheimer’s disease. Each year, more than 500,000 Americans die from this disease, making it the third leading cause of death in the United States. Traffic accidents, local military conflicts, some sports form a very numerous specific group of patients with craniocerebral injuries. Aim — to develop a phototherapeutic hardware complex for the prevention, treatment and rehabilitation of vascular, inflammatory, degenerative, post-traumatic and other brain diseases that are difficult amenable to medical treatment including pharmacoresistant indomitable pain syndromes. Materials and methods. The low-intensity electromagnetic radiation of the optical range of the spectrum and the magnetic field of a permanent magnet are physical factors that are used in phototherapeutic complex devices. Results. A phototherapy apparatus complex has been developed for the prevention, treatment and reha- bilitation of vascular, inflammatory, degenerative, post-traumatic and other brain diseases that are difficult to treat with drugs. The phototherapeutic hardware complex includes three devices: the modified photon-mag- netic apparatus Korobov-Posokhov “Barva-CNS/FM”, the photon-magnetic matrices A.Korobov - V.Korobov “Barva-Laryngologist/FM” and flexible photon-magnetic matrices A.Korobov - V.Korobov “Barva-Flex/24FM”. Conclusions. The presented clinical observations obtained using the developed phototherapeutic hard-ware complex for the treatment and prevention of diseases of the brain indicate high efficacy and safety of long-term low-intensity phototherapy in patients with dyscirculatory encephalopathy, with suffered ischemic and hemorrhagic strokes, with hypertensive disease.
