Introduction Laparoscopic surgery for colorectal cancer and liver tumors are accepted as alternative procedure to open surgery. However, few studies reported outcomes of simultaneous laparoscopic surgery of these two procedures. The aim of this study was to compare short-term outcomes between laparoscopic and open approach. Materials and methods Between June 2010 to December 2019, simultaneous laparoscopic cases were retrospectively matched (1:2) to open cases. Peri-operative and short-term outcomes were compared between both groups. Results Twelve patients in laparoscopic group were matched to 24 patients in open group according to age, gender, body mass index, american society of anesthesiologists physical status, preoperative laboratory data, number and size of liver metastases and extent of colorectal and liver resection, Most patients in each group had left-sided colon or rectal cancer and underwent wedge liver resection. The mean number of liver metastases was 1.3 vs 1.5 and size of liver metastases was 2.2 ± 1.4 vs 2.7 ± 1.1 cm in laparoscopic compared to open group. Estimated blood loss and length of hospital stay were significantly lower in laparoscopic group. However, operative time was significantly longer in laparoscopic group. Peri-operative complication was not significant difference between both groups and there was no mortality. Conclusion Simultaneous laparoscopic colorectal surgery and minor liver resection is feasible and safe. Laparoscopic approach has better peri-operative outcome in term of shorter length of hospital stay compared to open approach.

Background: Recommendations for candidates of bariatric surgery are available but they seldom give guidance to help a specific patient. The purpose of this study is to develop a new tool for determining the difficulty of laparoscopic bariatric surgery before surgery. Methods: This was a mixed method study. After a comprehensive search, semi structure interview and focus group discussions (FGD) with surgeons, and operating room personnel and department, initial draft of the questionnaire was prepared. We used Delphi method to achieve a group consensus of national and international surgeons. Finally, after several times of review and modification of the designed questions, a final questionnaire was prepared. Content and face validity, reliability and internal consistency (using Cronbach’s alpha) were determined.Results: Final questionnaire has 31 questions consist of I. Factors related to surgeon: experience(four questions), and exposure(one question), II. Factors related to patient: anatomy(eleven questions), and comorbidities(two questions), patient’s position(one question), and others(one question), III. Factors related to instruments and operation room, quantity(two questions), and quality(one question), IV. Factors related to other personnel: anesthesia team(six questions), and other personnel(two questions). Cronbach’s alpha was 0.98 for the whole questionnaire and between 0.86 and 0.97 for the four subscales. Conclusions: The psychometrics analysis indicated that it is valid and reliable for determining the difficulty of laparoscopic bariatric surgery. We recommend using this questionnaire for different settings either need to know difficulty of a bariatric surgery before a surgery or as an independent variable in various research projects.

Purpose: Minimally invasive oesophagectomy is a technically demanding procedure; thus, the learning curve of this procedure should be explored. Then the relatively mature minimally invasive oesophagectomy procedure should be contrasted with the open procedure. Methods: 214 consecutive patients underwent minimally invasive oesophagectomy were retrospectively reviewed. To evaluate the development of thoracoscopic-laparoscopic oesophagectomy and compare the mature minimally invasive oesophagectomy and open oesophagectomy (OE), we comprehensively studied the clinical and surgical parameters. The cumulative sum (CUSUM) plot was used to study the learning curve for systemic lymphadenectomy. Cox proportional hazards regression analysis was performed to evaluate the clinical factors affecting survival. Results: The bleeding volume, operation time, and postoperative mortality within 3 months significantly decreased after 20 patients. The rise point for the lymph nodes dissection number was visually determined to be at patient 57 in the CUSUM plots. Patients who underwent relatively mature thoracoscopic-laparoscopic oesophagectomy had better surgical data and short-term benefits than patients who underwent an open procedure. Cox proportional hazards regression analysis showed that the maximum diameter of the tumour cross-sectional area and the number of positive nodes had a significant influence on survival. Conclusions: The short-term benefits of thoracoscopic-laparoscopic oesophagectomy were suggested. There was no evidence that it is associated with a significantly better prognosis for patients with oesophageal cancer.
