Striae Distensae after Breast Augmentation

Context:Striae distensae are linear atrophic dermal scars covered with flat atrophic epidermis. They may cause disfigurement, especially in females. Many factors may cause striae distensae such as steroids, obesity, and pregnancy. Although there is no standard treatment for striae; many topical applications, peeling, and light and laser systems have been tried.Aims:To evaluate and compare the efficacy of fractional CO2 laser with intense pulse light in treating striae distensae.Subjects and Methods:Forty patients with striae distensae were recruited. Twenty of them were treated by fractional CO2 laser and 20 were treated with intense pulse light. Length and width of the largest striae were measured pre- and post-treatment. Patient satisfaction was also evaluated and graded. Patients were photographed after each treatment session and photos were examined by a blinded physician who had no knowledge about the cases.Results:Both groups showed significant improvement after treatments (P < 0.05). Patients treated with fractional CO2 laser showed significant improvement after the fifth session compared with those treated with ten sessions of intense pulsed light (P < 0.05) in all parameters except in the length of striae (P > 0.05).Conclusions:The current study has provided supportive evidence to the effectiveness of both fractional CO2 laser and intense pulse light as treatments for striae distensae. Fractional CO2 laser was found to be more effective in the treatment of striae distensae compared with intense pulse light.

Vitamin D (VD) plays a role in the skin regulation. Striae Distensae (SD) are manifestations of epidermal atrophy that occurs after tissue tearing due to overstretching or rapid growth. The objective of this study was to investigate the relation between serum VD and occurrence of SD in women who had undergone mammaplasty with silicone implants. A case-referent study was conducted. The blood values of 25-hydroxyvitamin D (25OHD) were measured before the surgery. For each patient postoperatively diagnosed with SD, four other participants submitted to the same surgery, without the development of SD, were enrolled as the healthy controls. 67 women with SD after the surgery entered the study. 268 formed the control group. In the serum of healthy controls 25OHD mean was 27 ng/mL, and SD cases presented 20 ng/mL (P = 0.01). Scarce values of VD have been observed in 56.71% of the cases presenting SD and in 39.91% without SD (P = 0.002). Chance of having VD values lower than 20 ng/mL amongst cases with SD is 2.38 (P = 0.0001). Lower serum levels of VD are linked to a higher occurrence of SD.

S tretch marks are manifestations of epidermal atrophy and result from tissue damage due to excessive tension [1, 2]. Stretch marks are one of the most common skin lesions for both women and men. The most frequent places of their locations are the abdomen, buttocks, thighs, chest, sleep, armpits and groin. Striae are divided according to the type and etiology on atrophic, stretch marks be-belt, linear stretch marks, red stretch marks, white striae, striae black and dark blue [3, 4]. Blue and black stretch marks are less common than other clinical types of stretch marks, especially among patients with dark skin phototypes due to the increasing saturation of melanin and in the foci [5, 6]. Due to its location in a particular anatomical region for stretch marks is characterized by the following pattern. Pragnacy stretch marks are predominantly localised in the abdominal area and on the breast; the adolescent stretch marks are localised on the thighs and in the lumbosacral region; nonpregnant young women get stretch marks mainly in the chest area and hips [5, 7, 8]. Most often striae are perpendicular to the lines of Langer, in directions with a minimum capacity of the skin to stretching [1, 2]. Striae represent linear atrophic scars, able despite the small area and, most often, the localization at closed clothing parts of the body, cause significant psychological difficulties and lower quality of life due to aesthetic concerns, and because of such subjective symptoms as itching [5, 9, 10, 11].
