Approximately 80% of psoriatic individuals experience pruritus, of varying intensity. This study evaluated the frequency of vulvar itching and burning and its influence on well-being in women with psoriasis. A total of 93 women were included in the study. Psoriasis severity was assessed according to the Psoriasis Area and Severity Index, the intensity of vulvar discomfort by visual analogue scale and depressive symptoms by Beck's Depression Inventory. On admission 41 (44.1%) women experienced vulvar discomfort, 18 (19.4%) itching, 10 (10.8%) burning and 13 (14.0%) both itching and burning sensations. Psoriatic lesions on the vulva were found in 22 (23.7%) women. No significant correlation was found between burning or itching intensity and global psoriasis severity (r = 0.19, p = 0.26). Patients with vulvar discomfort had psoriatic lesions on the vulva more often than women without discomfort (43.6% vs. 8.2%, p < 0.001). In addition, patients with vulvar discomfort more frequently demonstrated depressive symptoms (p < 0.05). We conclude that vulvar discomfort is an important clinical problem in women with psoriasis and should be taken into consideration during treatment.

Hintergrund: Psoriasis oder Schuppenflechte ist eine verbreitete Hautkrankheit, die durch rote Plaques mit hellgrauen Schuppen gekennzeichnet ist und mit beträchtlicher psychosozialer Belastung einhergeht. Sie ist in vielen Studien weltweit beschrieben worden, spezifische Daten für die Maghreb-Region (Algerien, Marokko und Tunesien) liegen jedoch nicht vor. Ziele: Die Beschreibung der Häufigkeit von Psoriasisneuerkrankungen und des epidemiologischen und klinischen Profils der Psoriasis im Maghreb. Methoden: Eine Psoriasisarbeitsgruppe für den Maghreb initiierte die EPIMAG-Studie, eine internationale, multizentrische epidemiologische Querschnitts-Beobachtungsstudie in Verbindung mit einer 2-wöchigen Psoriasis-Screening-Studie im Rahmen ärztlicher Konsultationen. Die Daten wurden mittels Fragebogen erhoben. Ergebnisse: Die analysierte Gesamtpopulation umfasste 373 bestehende und 326 neue Fälle von Psoriasis, beschrieben von 261 teilnehmenden Prüfärzten. Die Neuerkrankungshäufigkeit betrug 10,26/ 1000 Personen in Algerien, 15,04/1000 in Marokko und 13,26/1000 in Tunesien; somit lag die Häufigkeit für den gesamten Maghreb bei 12,08/1000 Personen. Das mittlere Alter aller 699 Psoriasispatienten lag bei 46 Jahren, der mittlere BMI betrug 26,6 und 55,7% der Patienten waren Männer. Zwei Drittel aller Patienten hatten nie geraucht, und 85,0% hatten nie Alkohol getrunken. Die Hälfte war dunkler Hautfarbe, und bei 28,6% lag Psoriasis in der Familienanamnese vor. In drei Viertel der Fälle war die Psoriasis lokal begrenzt, bei 85,8% handelte es sich um Psoriasis vom Plaque-Typ, bei über 70,0% ging sie mit Pruritus einher. Die häufigsten Auslöser von Schüben oder «Flares» waren Stress (79,4%) und ein Wechsel der Jahreszeiten (43,1%). Die meisten Teilnehmer wandten topische Therapien an, und die Prüfärzte stuften diese in über der Hälfte der Fälle als «teilweise wirksam» ein. Eine sekundäre Analyse der Patienten mit vorbestehender Psoriasis ergab, dass es sich in 73,2% der Fälle um eine schwere Psoriasis handelte und die Lebensqualität bei 40,1% der Patienten stark beeinträchtigt war. Die mittlere Anzahl Fehltage in der Schule bzw. bei der Arbeit betrug 3,2 (± 12,1) Tage in 6 Monaten. Schlussfolgerungen: Unsere Studie liefert neue Informationen zur Epidemiologie und Charakterisierung der Psoriasis im Maghreb und verdeutlicht den Verbesserungsbedarf bei der Früherkennung und Behandlung von Psoriasis in der Region. Die Daten werden dazu beitragen, das Psoriasismanagement zu optimieren und adäquates gesundheitspolitisches Handeln auf nationaler Ebene sicherzustellen. Introduction: Dermatological problems often present mood disorders, depression, anxiety disorders and psychological conditions relevant to psychosomatic conditions. Seborrheic dermatitis and psoriasis, both characterized by chronic inflammation of skin, have been linked to emotional states. The aim of our study was to compare the two emotional profiles of patients with seborrheic dermatitis and psoriasis, in order to evaluate, quantitatively and qualitatively, the emotional picture in the two groups of disease. Methods: The study group included 33 patients affected by seborrheic dermatitis (21 males and 12 females, age range 18 -67 years, mean age 39.85 ± 15.45 years) and 36 other patients affected by psoriasis (22 males and 14 females, age range 18 -67 years, mean age 41.11 ± 13.72 years). The protocol included clinical assessment of the skin disorder and psychological assessment of emotional profile using the test Profile of Mood States (POMS) and the Toronto Alexithymia Scale TAS-20. An exploratory factorial analysis was carried out for the POMS factor D (depression) to evaluate the composition of the items found in the two types of skin disorders. Results: The comparison between patients with psoriasis and patients with seborrheic dermatitis in relation to the TAS score and the different aspects of mood and showed a significant difference concerning factor D (depression) of the POMS. Conclusions: This study shows the correlation between depression and two skin disorders, seborrhoeic dermatitis and psoriasis. In both cases, the clinical picture falls under the definition of "depression due to a general medical condition". However, qualitative differences make the psychologic frame of psoriatic patients close to that of "major depression", while in patients affected by seborrheic dermatitis the psychologic frame is more similar to that of "minor depression" (DSM IV).
