Enabling is a concept central to health promotion. It is perceived as a mechanism that can help people gain control over determinants of health. Little is known, however, about enabling among policy-makers and professionals. This case study investigates enabling among policy-makers and professionals who engaged in a specific participatory approach, cooperative planning. We define 'enabling' as creating action situations that allow policy-makers and professionals to (i) build individual capacities for health promotion and to (ii) apply these capacities to concrete organizational and political action at the institutional level. This case study followed policy-makers and professionals as they participated in a local physical activity promotion action research project in Germany. We conducted a secondary analysis of qualitative data gathered in that project (2005-2011). Methods included participant observation, document analysis, focus groups and qualitative interviews. All data were revisited for the case study and analyzed using qualitative content analysis. Findings include examples of enabling among policy-makers and professionals related to the cooperative planning process. Individual capacities were developed in perceived project roles, interactions with target groups and decision-making procedures. Findings also demonstrated municipal policy changes. Access to physical activity infrastructures improved, and an intersectoral job position was funded to support physical activity promotion among target group participants. Findings were analyzed using a model that links cooperative planning with a framework on policy change from a political science perspective. We conclude that cooperative planning might be a pathway to negotiated agreements that foster systematic enabling and health-promoting policy change.

Physical inactivity is most commonly found in socially vulnerable groups. Dutch policies target these groups through community-based health-enhancing physical activity (CBHEPA) programs. As robust evidence on the effectiveness of this approach is limited, this study investigated whether CBHEPA programs contribute to an increase in and the maintenance of physical activity in socially vulnerable groups. In four successive cohorts, starting at a six-month interval, 268 participants from 19 groups were monitored for twelve months in seven CBHEPA programs. Data collection was based on repeated questionnaires. Socio-economic indicators, program participation and coping ability were measured at baseline. Physical activity, health-related quality of life and on-going program participation were measured three times. Self-efficacy and enjoyment were measured at baseline and at twelve months. Statistical analyses were based on a quasi-RCT design (independent t-tests), a comparison of participants and dropouts (Mann-Whitney test), and multilevel modelling to assess change in individual physical activity, including group level characteristics. Participants of CBHEPA programs are socially vulnerable in terms of low education (48.6%), low income (52.4%), non-Dutch origin (64.6%) and health-related quality of life outcomes. Physical activity levels were not below the Dutch average. No increase in physical activity levels over time was observed. The multilevel models showed significant positive associations between health-related quality of life, self-efficacy and enjoyment, and leisure-time physical activity over time. Short CBHEPA programs (10–13 weeks) with multiple trainers and gender-homogeneous groups were associated with lower physical activity levels over time. At twelve months, dropouts' leisure-time physical activity levels were significantly lower compared to continuing participants, as were health-related quality of life, self-efficacy and enjoyment outcomes. BMI and care consumption scored significantly higher among dropouts. In conclusion, Dutch CBHEPA programs reach socially vulnerable, but not necessarily inactive, groups in terms of socio-economic and health-related quality of life outcomes. Our findings suggest that CBHEPA programs particularly contribute to physical activity maintenance in socially vulnerable groups, rather than to an increase in physical activity behaviour over time.

ZusammenfassungAnsätze der kommunalen bewegungsbezogenen Gesundheitsförderung (kbG) werden empfohlen, um körperlicher Inaktivität in der Bevölkerung entgegenzuwirken. Der vorliegende Artikel hat zum einen das Ziel, eine systematische Übersicht zur kbG in Deutschland darzustellen. Zum anderen sollen in diesem Bereich Good-Practice-Projekte identifiziert werden, die als Orientierungsrahmen für die Umsetzung und Verbreitung von kbG genutzt werden können. Im ersten Schritt wurden Projekte über eine Suche in 4 wissenschaftlichen Datenbanken sowie 21 Projektdatenbanken identifiziert und Daten zu „Bundesland“, „Berücksichtigung gesundheitlicher Chancengleichheit“ und „Umsetzung im städtischen oder ländlichen Raum“ extrahiert. Zudem wurden eingeschlossene Projekte in verschiedene Ansätze im Setting Kommune unterteilt. Im nächsten Schritt wurden Projekte mit einer dokumentierten Prozess- und/oder Ergebnisevaluation anhand von Qualitätskriterien in den Bereichen Konzipierung, Implementierung und Evaluation bewertet. Projekte, die mindestens 50 % der Qualitätskriterien erfüllten, wurden als Good-Pratice-Projekte ausgewählt. Insgesamt wurden 240 Projekte der kbG erfasst. Im ländlichen Raum wurden primär umweltbezogene Ansätze und im städtischen Raum vermehrt Mehrkomponentenansätze sowie Angebote und Veranstaltungen umgesetzt. 45 Projekte wiesen eine ausreichende Dokumentation der Prozess- und/oder Ergebnisevaluation auf, wovon 17 Projekte als Good-Practice identifiziert wurden. Optimierungspotenzial besteht bei der Adressierung von gesundheitlicher Chancengleichheit, einer aktiven Beteiligung von Menschen in schwierigen Lebenslagen über den gesamten Projektzyklus sowie der Implementierung von Mehrkomponentenansätzen. Dies erfordert Förderprogramme, die strukturbildende Good-Practice-Projekte ermöglichen und insbesondere deprivierte Kommunen erreichen. Ein von Akteur:innen aus Wissenschaft, Praxis und Politik gemeinsam verabschiedeter Standard zur Anwendung der Qualitätskriterien und einer damit einhergehenden Legitimation für die Bezeichnung als Good-Practice könnte einen Mehrwert für die strategische Weiterentwicklung der kbG in Deutschland bieten.
