Stratified, randomized, double-blind comparison of intravenous ondansetron administered as a multiple-dose regimen versus two single-dose regimens in the prevention of cisplatin-induced nausea and vomiting.

Abstract: A 32-mg single dose of ondansetron is more effective than a single 8-mg dose and is at least as effective as the standard regimen of 0.15 mg/kg times three doses in the prevention of cisplatin-induced acute emesis.

“…5-HT 3 -receptor antagonists have become the agents of choice in preventing acute CINV following both moderately and highly emetogenic chemotherapy [11,51]. Complete response and total control rates seen with specific drugs range from 60%-80% for moderately emetogenic chemotherapy [4,[52][53][54], 40%-60% for cisplatin-containing therapy [4,33,55,56], and 25%-60% for high-dose cisplatin regimens [33,35,47,57,58].…”

“…Recent studies showed that a single 8 mg i.v. dose of ondansetron was as effective as the more complicated regimens (Beck et al, 1992;Seynaeve et al, 1990). Although our study is based on a small number of patients, the complete response (67%), complete plus major response (89%) and the failure (0%) rates obtained in this trial with a single 8 mg i.v.…”

“…Intravenous ondansetron, given either in repeated doses or as a continuous infusion, antagonises vomiting associated with cisplatin treatment (Cubeddu et al, 1990;Beck et al, 1992;Seynaeve et al, 1992). These regimens have high pharmacy and nursing costs, and often lengthen the duration of hospitalisation.…”

“…(0.015, 0.15 and 0.3 mg kg 71 ) indicated a trend towards increased efficacy with higher doses (complete response rates were 15, 46 and 58% respectively), though only Table 1 Emetogenic potential and onset of emetic response following administration of cisplatin and cyclophosphamide the 0.015 mg kg 71 dose was shown to be significantly different from the other dose levels (Grunberg, 1992). Further studies have reported significantly superior efficacy with a single dose on ondansetron, 32 mg, compared with a single 8 mg dose (Beck et al, 1992;Tsavaris et al, 2001). Moreover, studies comparing granisetron, 40 and 160 mg kg 71 , show a trend towards increased efficacy at the higher dose Soukop, 1994); 24 h after administration of chemotherapeutic agents, 81% of granisetron, 160 mcg kg 71 , were emesis-free compared with 75% receiving granisetron 40 mg kg 71 .…”
