Strategies for early melanoma detection: Approaches to the patient with nevi

Abstract: Given its propensity to metastasize, and lack of effective therapies for most patients with advanced disease, early detection of melanoma is a clinical imperative. Although there are no non-invasive techniques for definitive diagnosis of melanoma, and the "gold standard" remains biopsy with histologic examination, a variety of modalities may facilitate early melanoma diagnosis and the detection of new and changing nevi. This article reviews general clinical principles of early melanoma detection, and various m… Show more

“…Finally, sequential follow-up of melanocytic lesions, although routine in medical practice, could be readily implemented via temporally separated remote assessments. As is well recognised, detecting meaningful change in melanocytic lesions may, in some cases, be the only way of reliably diagnosing melanoma [8]. …”

“…While the prognosis for patients with advanced disease remains poor, melanoma can be cured if treated early [3,4,5,6,7]. Dermatologists and other health professionals have therefore promoted a number of strategies to increase the rates of early detection of melanoma [7,8], including, but not limited to: educational campaigns about sun safety [9] and the importance of skin self-examination (SSE) [10], a user-generated web-based dermoscopy image archive as a contribution to E-learning and exchange of knowledge [11], regular screening examinations by primary health care physicians or dermatologists, and the application of innovative clinical tools such as dermoscopy and total body imaging [8,9,10,11,12,13]. …”

“…However, melanoma is an aggressive malignancy that tends to metastasize beyond its primary site (7,12). Once melanoma is advanced, surgery is no longer sufficient and the disease becomes more difficult to treat (7,(12)(13)(14). Long-term prognosis after metastasis is grim; median survival with treatment, including treatment with immunotherapeutics like Ipilimumab, ranges from 8 to 12 months (6,13,14).…”

Long-term prognosis after metastasis is grim; median survival with treatment, including treatment with immunotherapeutics like Ipilimumab, ranges from 8 to 12 months (6,13,14). However, more recently developed combined immunotherapeutic treatments with radiation can improve survival further to several years (13).…”

“…Besides HFUS, confocal microscopy and OCT have emerged in recent years as imaging modalities providing real-time evaluation of cutaneous tissue [10,12,13]. Reaching a penetration depth of about 1 mm, layers such as the epidermis and the dermis, adnexal structures, and blood vessels can be well distinguished by OCT [14].…”
