Strangulated Appendix within a Trocar Site Incisional Hernia Following Laparoscopic Low Anterior Rectal Resection. A Case Report

Abstract: Appendix strangulation caused by an incisional trocar site hernia following laparoscopy is an extremely rare entity. A 63-year-old man was admitted to our department with continuous abdominal pain and irreducible minimal swelling in the right abdomen. His surgical history was significant for rectal cancer one year previously. The patient had undergone a laparoscopic low anterior rectal resection with construction of a protective temporary end ileostomy (pT1, pN0, G2, M0). The ileostomy was reversed 3 months af… Show more

“…Although they usually contain omental fat or small bowel, three cases of the vermiform appendix being present in a symptomatic port-site hernia have been reported [1, 13, 14]. Port-site hernias in general are reported to occur with an incidence of 0.74%, depending on the type of laparoscopic surgery performed.…”

“…With the introduction of laparoscopy and thoracoscopy, herniation or evisceration of intra-abdominal organs including the appendix has been one of the many complications of the same. Strangulation of the appendix caused by an incisional trocar site hernia following laparoscopy was reported by Menenakos et al [4].…”

“…[11] Two previous cases of the appendix appearing in a PSH have been described, one of which presented in a similar manner and location to our case. [4] Standard appendectomy through the hernial defect is the treatment of choice if access permits, otherwise consideration should be given to conversion to a transabdominal route for improved access or if appendicitis has generated significant wound contamination. Mesh hernioplasty is safe if no wound contamination occurs.…”
