Background School violence continues to afflict our educational institutions. In response, an institutional initiative was launched to train educators and school support staff in life-saving skills aimed at hemorrhage control. Methods The American College of Surgeons Committee on Trauma “Stop the Bleed” (STB) Program was promoted as a quality improvement initiative to schools within the geographic catchment area of this Level I Trauma Center. Participants were given the opportunity to take precourse, and postcourse confidence inventories using a Likert Scale. Statistical analysis of the 324 precourse to postcourse evaluations measuring change in confidence was used to evaluate improvement in readiness of school systems to respond in mass casualty incidents. Results Students enrolled in the STB Program were offered the opportunity to assess their confidence precourse and postcourse in reference to 7 questions. Precourse and postcourse Likert Scale inventories were compared and analyzed to assess the strength of the improvement in confidence using Student’s t-test, where P < .05 is statistically significant. Students demonstrated improvement ( P < .006) that was statistically significant across all 7-question relating to enhance confidence postcourse compared with the precourse. Discussion This STB quality initiative has demonstrated a statistically significant improvement in the confidence of teachers and school personnel to render lifesaving care in the event of a mass casualty or isolated incident of life-threatening hemorrhage. These results support the validity of the training in making a difference in this subpopulation of responders.

Efficacy of an 8-week Bedside Ultrasound Training Program for Surgical Residents in the Intensive Care Unit

BackgroundBedside ultrasound has become one of the most important non-invasive and readily available diagnostic tools especially for critically ill patients. However, the current ultrasound training program is not standardized and is mostly unavailable to all surgical residents equally. The aim of this study was to assess and evaluate the effectiveness of our new training program in bedside ultrasound for surgical residents.MethodsPostgraduate residents (years 1 to 4) from the department of general surgery in a tertiary hospital attended the newly designed, 8-week training course in bedside ultrasound for critically ill patients at the surgical intensive care unit. Didactic and experimental lectures in basic ultrasound physics and machine usage were delivered, followed by daily hands-on ultrasound training to patients. Each participant prospectively documented their ultrasound findings and completed self-assessment of ultrasound skills using a Likert scale.Results44 residents were enrolled in the current study and only 36.4% of them were previously exposed to bedside ultrasound experience. Following the course completion, the proficiency levels and the objective structured assessment of ultrasound skill (OSAUS) scores showed significant improvement in all elements (P < 0.001). The mean differences in pre- and post-course score between post-graduate years were recorded, except for proficiency in peritoneal cavity (P = 0.163). Post-hoc analysis revealed that post-graduate year 2 residents showed a higher improvement in most elements. The training program showed improvement in post-course scores, whether or not residents had previous experience.ConclusionsOur results revealed that the knowledge and confidence of surgical residents in bedside ultrasound could be improved via short-term and impact training curriculum. The authors believe that such education should be encouraged for all surgical residents to enhance competency in performing bedside ultrasound evaluation of critically ill patients.

Aim The Bleeding Control Basic (BCon) course was developed by the American College of Surgeons to teach laypeople and health‐care providers (HCPs) how to stop life‐threatening bleeding. The first BCon course in Japan was held for HCPs in July 2018. Our study aimed to evaluate the utility of the course, the satisfaction and confidence level of the HCPs that participated, and their experience with using vascular tourniquets. Method The BCon participants were asked to complete a survey after the BCon courses from December 2018 to December 2019. These participants included different types of HCPs (physicians, nurses, and emergency medical technicians). After the course, the participants were asked to evaluate: (i) the perceived utility of the course, (ii) their satisfaction with the course, (iii) their confidence in the techniques that they learned in the course, (iv) their experience of using tourniquets in eight specific areas using a 10‐point Likert scale. Results A total of 163 HCPs, including 108 physicians, 27 nurses, and 28 emergency medical technicians completed the BCon course. The respondents rated the course highly, showing an average value of approximately 9 for each item for perceived utility, satisfaction, confidence, and experience in using tourniquets. In particular, nurses rated the overall activity more highly than physicians (P < 0.05). Conclusion The BCon course and tourniquets were well‐received by all types of HCPs in Japan.
