Background Stigma refers to the discrediting, devaluing, and shaming of a person because of characteristics or attributes that they possess. Generally, stigma leads to negative social experiences such as isolation, rejection, marginalization, and discrimination. If related to a health condition such as mental illness, stigma may affect a person’s illness and treatment course, including access to appropriate and professional medical treatment. Stigma has also been reported to affect patients’ families or relatives, along with professionals who work in mental healthcare settings. Stigma is strongly influenced by cultural and contextual value systems that differ over time and across contexts. However, limited information is available on how types of stigma are experienced by patients with mental illness and mental health nurses in Indonesia. Method We explored the stigma-related experiences of 15 nurses and 15 patients in Indonesia. The study design and analysis of interview data were guided by deductive (directed) content analysis. Results Five themes emerged. Four themes were patient-related: personal/patients’ stigma, public/social stigma, family stigma, and employment stigma. The fifth theme related to stigma toward healthcare professionals working with patients with mental illnesses, which we categorized as professional stigma. Conclusions This study has achieved a deep understanding of the concept of stigma in the Indonesian context. This understanding is a prerequisite for developing appropriate interventions that address this phenomenon and thereby for the development of mental health services in Indonesia. This study may also be transferable to other countries that share similar cultural backgrounds and adhere to traditional and religious value systems.

Penderita gangguan jiwa saat ini banyak menerima stigma, tidak hanyak berdampak pada penderita saja, melainkan juga berdampak pada persepsi dan sikap keluarga dalam merawat penderita gangguan jiwa. Tujuan penelitian adalah untuk mengetahui bagaimana persepsi keluarga terhadap anggota keluarga dengan gangguan jiwa. Penelitian ini menggunakan metode kualitatif dengan desain fenomenologi. Sampel pada penelitian berjumlah 7 partisipan, diambil menggunakan teknik purposive sampling dengan kriteria inklusi keluarga yang memiliki anggota keluarga dengan gangguan jiwa dan yang berhubungan langsung dengan ODGJ minimal lamanya 2 tahun. Analisa data menggunakan metode Colaizzi. Hasil penelitian yaitu terdapat 7 tema yang menggambarkan persepsi keluarga, yaitu 1) Keluarga mengetahui tanda dan gejala yang dimiliki oleh anggota keluarga dengan gangguan jiwa. 2) ODGJ memiliki perilaku positif dan negatif terhadap keluarga dan masyarakat. 3) Keluarga dapat terhindar dari gangguan jiwa dengan memahami faktor penyebab gangguan jiwa. 4) Keluarga memiliki fungsi untuk mencegah dan merawat anggota keluarga dengan gangguan jiwa. 5) Keluarga memiliki persepsi yang berbeda-beda terhadap orang dengan gangguan jiwa. 6) Keluarga memberikan sikap caring kepada anggota keluarga dengan gangguan jiwa. 7) Keluarga memiliki harapan yang positif terhadap ODGJ dan masyarakat agar anggota keluarga dengan gangguan jiwa bisa pulih.

Peer stigma, consequences, and anti-stigma strategies in adolescents with mental disorders: a qualitative investigation

Purpose This paper aims to investigate the local perspectives, based on culture and beliefs about peer stigma, consequences and strategies to reduce mental health stigma among adolescents in Indonesia. Design/methodology/approach This study included 30 participants, including students, teachers, health professionals and lecturers. Two sessions of focus group discussions and 14 sessions of in-depth interviews were performed to collect the data. A qualitative content analysis was carried out, and some related themes and sub-themes were explored. Findings This study found that peer stigma was prevalent and negatively impacted the psychological, social and intellectual development of adolescents with mental disorders (MD) in Indonesian society. Five strategies to tackle peer stigma were proposed, including increasing mental health literacy (MHL), reducing the harmful effects of stigma, fostering a supportive social environment, expanding access to and improving the quality of mental health care and advocating for public health policies. Research limitations/implications More qualitative research on the experiences and perspectives of victims, perpetrators and society concerning mental health stigma in adolescents is required to fully understand why stigma arises, what causative factors and repercussions it has, as well as how to deal with it. Practical implications Peer stigma still exists among adolescents. It has negative impacts on their mental and psychological well-being. MHL, social environment, the harmful effects of stigma, as well as mental health access and policy were concerns of this study. A school is a place where adolescents spend most of their social time. Schools play a vital part in the stigma-reduction program. Originality/value Mental health stigma among adolescents with MD is an issue that has potentially hindered mental health recovery. Anti-stigma intervention based on local perspectives is valuable in increasing the effectiveness and acceptance of interventions.
