Stiff person syndrome (SPS) complicated by respiratory failure: successful treatment with rituximab

Abstract: Dear Sirs, SPS is characterized by axial rigidity, progressive stiffness, and spontaneous reflex-or action-induced painful spasms of the paraspinal, abdominal and occasionally proximal leg muscles associated with a lumbar hyperlordosis. Most patients have anti-glutamic acid decarboxylase (GAD) antibodies in both serum and cerebral spinal fluid (CSF) with additional evidence of autoimmune disease, including thyroid disorders, insulin-dependent diabetes mellitus (IDDM), adrenal insufficiency, vitiligo, pernicious… Show more

“…Rituximab effectively treated SPS, even in cases with complications, such as dysthyroid ophthalmopathy and respiratory failure [128][129][130]. In a most recent report of a case study, rituximab in combination with tizanidine improved SPS along with the immunomodulatory effect previously caused by benzodiazepines [131].…”

“…Antibodies to the glycine receptor have been identified in patients with PERM [307,308]. Patients with SPS, and especially PERM, may develop respiratory failure and are at risk for sudden cardiac death [309].…”

“…Because SPS is an antibody mediated disorder, it would be expected that plasmapheresis and rituximab would be reasonable treatments, and both have been used with reports of success [309,312].…”
