Background: Cutaneous Adverse Drug Reaction (CADR) is a reaction that can cause changes in the structure, the function of the skin, and mucous membranes caused by drug use. One of the risk factors of CADR is a comorbid disease. Objective: to determine the relationship between comorbid diseases and the severity of Cutaneous Adverse Drug Reaction (CADR) at Dr. Saiful Anwar Hospital, Malang. Methods: This study used secondary data taken from medical records of skin and genital patients at Dr. Saiful Anwar Hospital Malang in 2018-2020 and was analyzed using SPSS software version 23 with the Chi-Square test method to determine the significance of the relationship between variables. Result: This study was conducted on 167 patients with mild CADR (52.7%) more than severe CADR (47.3%). The most frequently experienced mild and severe CADR was exanthematous drug eruption and DRESS. In addition, there were also 68 CADR patients with the comorbid disease (40,7 %). Seen from the statistical tests performed, obtained p-value = 0.074 and Odds Ratio (OR) = 1.861. Judging from the p-value, it can be concluded that there is no significant relationship between comorbid disease and the severity of CADR (p > 0.05). Despite that, when viewed from the OR results, it shows that patients with comorbid diseases have a statistically higher risk of 1.861 times higher of experiencing the mild type of CADR than the severe type. Conclusion: One to suggest for this research is to collect medical record data within a longer time span. It is expected that the data is complete and as needed so that it can support the shortcomings in this study.

Latar Belekang: Berbagai penyakit kulit dan kelamin telah dihubungkan dengan infeksi HIV. Penyakit tersebut dapat secara spesifik berhubungan dengan HIV, namun bisa juga merupakan penyakit umum namun dengan manifestasi yang lebih parah dan lebih sulit diobati. Tujuan: untuk mengetahui karakteristik penyakit kulit dan kelamin pada pasien dengan infeksi HIV di Rumah Sakit Umum Daerah Buleleng. Metode: Penelitian ini menggunakan pendekatan studi potong lintang yang dilakukan pada periode bulan Mei 2023 hingga Juli 2023. Diagnosis infeksi HIV dibuktikan dengan pemeriksaan laboratorium, sementara itu diagnosis penyakit kulit dan kelamin ditegakkan secara klinis oleh dokter spesialis kulit dan kelamin, dan apabila diperlukan dilakukan pemeriksaan penunjang. Hasil: Terdapat 28 pasien HIV dengan keluhan penyakit kulit dan kelamin. Jumlah pasien laki-laki dibanding perempuan adalah 2,1:1 dengan dominasi pada kelompok umur 31-40 tahun. Sebagian besar dari pasien berada pada stadium 4 dan rute penularan terbanyak adalah karena seks bebas dan homoseksual. Terdapat 49 diagnosis di bidang kulit dan kelamin dari 28 pasien yang diperiksa. Karakteristik penyakit kulit dan kelamin pada pasien adalah 57,1 % diagnosis penyakit infeksi, sisanya adalah penyakit non infeksi. Kesimpulan: Penyakit infeksi diantaranya adalah infeksi bakteri (sifilis, ektima, granuloma inguinale, bartholinitis dan impetigo krustosa), infeksi virus (herpes zoster, kondiloma akuminata, moluskum kontagiosum dan herpes labialis) dan infeksi jamur (kandidiasis oroesofageal, kandidiasis oral dan angular cheilitis). Sementara itu, untuk kasus non infeksi terdiri dari dermatitis seboroik, papular pruritic eruption dan erupsi obat.

Co-trimoxazole is a sulfonamide fixed dose combination antibiotic which is effective and widely use in treatment and prevention of opportunistic infections in people living with HIV/AIDS, but can be potentially associated with adverse drug reactions including drug hypersensitivity reactions such as Steven Johnson syndrome. PLWHA individuals have up to a 1000-fold increased risk of drug hypersensitivity reaction compared to the general population, which is associated with drug exposure, immune dysregulation, and concurrent infections. Here we present a case report of a 59-year-old man with HIV who experienced Stevens-Johnson Syndrome after taking co-trimoxazole for 18 days. Therefore, we would like to remind you to be more careful when giving cotrimoxazole to PLWHA because it can take time for a hypersensitivity reaction to occur, also in a few cases, it can occur immediately.
