Stereotactic ventrolateralis thalamotomy for medically refractory tremor in post-levodopa era Parkinson's disease patients

Abstract: Thirty-six patients with Parkinson's disease and medically refractory tremor underwent stereotactic ventrolateralis thalamotomy at the Mayo Clinic between 1984 and 1989. All patients had been or were being treated with carbidopa/levodopa but with unsatisfactory tremor control. Modern stereotactic techniques, including microelectrode recording, were used to treat 36 patients, of whom 31 (86%) had complete abolition of tremor and three patients (5%) had significant improvement. Tremor recurred in two patients wi… Show more

“…Two studies employed CT scanning for at least part of the study 16,18 and the same two employed microelectrode or semi-microelectrode recording for physiologic localization. These methods seem important for proper localization within the thalamus, but controlled observations on this point are not available.…”

“…13,14 For thalamotomy, 18 articles were found, but only four studies met criteria. [15][16][17][18] For pallidotomy, of the 40 identified papers, 22 were included. 14, Of the 38 articles identified for DBS of the thalamus, 16 articles met criteria.…”
