Stereotactic robotic body radiotherapy for patients with oligorecurrent pulmonary metastases

Abstract: Background: Our aim is to report treatment efficacy and toxicity of patients treated by robotic (Cyberknife®) stereotactic body radiotherapy (SBRT) for oligorecurrent lung metastases (ORLM). Additionally we wanted to evaluate influence of tumor, patient and treatment related parameters on local control (LC), lung and distant progression free-(lung PFS/Di-PFS) and overall survival (OS). Methods: Consecutive patients with up to 5 ORLM (confirmed by FDG PET/CT) were included in this study. Intended dose was 60Gy … Show more

“…The ECOG index ranged from 0 to 3, with only one study reporting ECOG for three patients [ 16 ]. Oligometastatic disease was defined as one to five metastases in eight articles [ 17 , 18 , 19 , 20 , 21 , 22 , 23 , 24 ], one to four metastases in one article [ 25 ], one to three metastases in three articles [ 26 , 27 , 28 ], and six articles did not specify any definition [ 16 , 29 , 30 , 31 , 32 , 33 ].…”

“…Surgery was used at least once before SBRT in 164 patients, but no thermoablation before SBRT was identified. Only two authors specified surgery for metastases, which represented 44 patients [ 18 , 21 , 22 , 24 , 25 , 27 , 30 , 33 ]. The 2-year LC, PFS, and OS ranged from 31% to 93%, from 10% to 38%, and from 40% to 85%, respectively.…”

“…Visceral metastases (VM) are the most common metastases from CRC that are treated with SBRT [16] and much of the literature to date has focussed on either liver or lung metastases [17] . In the SABR-COMET trial [7] most patients had visceral metastases, while only 3 (6%) of metastases treated with SBRT were in lymph nodes.…”

“…However, there remains considerable rates of acute and late toxicities of the lung, chest wall, esophagus, and bronchial tree, with at least 8 series reporting fatal grade 5 toxicities. [12][13][14][15][16][17][18][19] Correspondingly, there is a scarcity of data in regard to dose-volume constraints or guidelines for safe reirradiation in this setting.…”
