Stereotactic Placement of Ventricular Catheters: Does It Affect Proximal Malfunction Rates?

Abstract: Background: The majority of ventriculoperitoneal (VP) shunt malfunctions are due to proximal catheter failure. Ideal placement of Ommaya reservoirs is desired to avoid toxicity from intraparenchymal chemotherapy infusion. Objective: To determine whether stereotactic placement of ventricular catheters decreases the rate of Ommaya reservoir complications and the rate of proximal VP shunt failure. Methods: Under institutional review board approval, areview of a prospectively collected database was doneidentifying… Show more

“…Much of this time was committed prior to incision, placing the patient in cranial fixation and registering the navigation system. This finding, which has been previously reported, 13 along with the cost of compatible preoperative imaging and the approximately $400 committed to disposable intraoperative materials, results in an increased cost to these procedures when neuronavigation is used.…”

“…Using rigid cranial fixation image guidance, Sampath et al illustrated that by using VP shunting with image guidance, time in the operative room was significantly increased but actual operative time was similar to published series using the freehand technique. 21 All shunts were placed with ideal end points in 1 pass. The placement of VP shunts with image guidance shows increased accuracy and decreased proximal failures.…”

“…Even with the use of fluoroscopic guidance for this indication, up to 4.7% of catheters can be malpositioned [9]. Studies of stereotactic catheter insertion for Ommaya reservoirs have reported complication rates from 0 to 16% [3,4,9,10,11,12]. Though two series of frameless placement of catheters for Ommaya reservoirs have been published, the growing popularity of frameless systems warrants the present direct comparison [3,4].…”
