Stereotactic body radiotherapy with or without external beam radiation as treatment for organ confined high-risk prostate carcinoma: a six year study

Abstract: BackgroundStereotactic Body Radiotherapy (SBRT) has excellent control rates for low- and intermediate-risk prostate carcinoma. The role of SBRT for high-risk disease remains less studied. We present long-term results on a cohort of patients with NCCN-defined high-risk disease treated with SBRT.MethodsWe retrospectively studied 97 patients treated as part of prospective trial from 2006–2010 with SBRT alone (n = 52) to dose of 35–36.25 Gy in 5 fractions, or pelvic radiation to 45 Gy followed by SBRT boost of 19–2… Show more

“…According to several other studies, gamma analysis to detect 1% of DD and 1 mm of DTA is still controversial for the following reasons: (a) dosimetric errors and statistical fluctuations are dominant for the 1%/1mm criterion, and (b) the acceptable level of %GP for the 1%/1mm criterion should differ from those values for the 3%/3mm and 2%/2mm criteria 36 , 37 , 38 . Thus, a careful approach to gamma analysis with the 1%/1mm acceptance criterion should be taken.…”

“…In situations like this, the AAA of Eclipse has been shown to underpredict dose5, 20 and CCCS overpredict dose,21 both relative to gold standard Monte Carlo calculations. These publications are consistent with local observations whereby Mobius3D systematically calculates more dose to lung targets than Eclipse , where the reality is most likely to be somewhere in between.…”

“…Recent improvements in the technology of 4‐Dimensional Computed Tomography (4DCT) image binning,15, 16, 17, 18 respiratory motion monitoring,18, 19, 20, 21, 22, 23 functional imaging correlation,24, 25, 26 and faster imaging techniques have resulted in several delivery methodologies to decrease the impact of lung motion. Another approach to reducing the impact is to minimize the motion itself by incorporating compression belts27 to restrict diaphragm contraction and expansion, implementing breath hold techniques23, 28, 29, 30 or gating the treatment by restricting delivery to particular components of the breathing cycle 14, 20, 25, 31, 32.…”
