STereotactic Arrhythmia Radioablation (STAR): the Standardized Treatment and Outcome Platform for Stereotactic Therapy Of Re-entrant tachycardia by a Multidisciplinary consortium (STOPSTORM.eu) and review of current patterns of STAR practice in Europe

Abstract: The EU Horizon 2020 Framework-funded Standardized Treatment and Outcome Platform for Stereotactic Therapy Of Re-entrant tachycardia by a Multidisciplinary (STOPSTORM) consortium has been established as a large research network for investigating STereotactic Arrhythmia Radioablation (STAR) for ventricular tachycardia (VT). The aim is to provide a pooled treatment database to evaluate patterns of practice and outcomes of STAR and finally to harmonize STAR within Europe. The consortium comprises 31 clinical and r… Show more

“…Cardiac radioablation (CR) recently appeared as a promising technique for treatment of refractory ventricular tachycardia (VT). [1][2][3] Also known as Stereotactic arrhythmia radiotherapy (STAR), 4 or cardiac stereotactic body radiotherapy (cardiac SBRT), 2 this treatment consists in delivering a high radiation dose (20 to 25Gy), in a single treatment session, to the myocardium tissues identified as the source of the VT. Patients suffering from VT undergo a series of treatments, including medications, implantation of an implantable cardioverter-defibrillator (ICD), and invasive catheter radiofrequency ablation (CRF), that constitutes the standard medical care.…”

“…Deep intramural areas or subepicardial locations, especially in the vicinity of structures such as coronary arteries, make effective ablation difficult which results in VT recurrence in about 20-50% of the patients. 125 Cardiac stereotactic body radiation therapy (SBRT), called STereotactic Arrhythmia Radioablation (STAR), is a promising new therapeutic modality for refractory VT patients without additional interventional options. 125 In a 2017 report of 5 patients who had 6577 episodes of VT in the 15 patient-months before treatment, after a 6-week blanking period, there were 4 episodes of VT post-STAR during the next 46 patient-months, a relative reduction of 99.9%.…”

“…Currently, several prospective clinical trials aimed at investigation of short- and long-term morbidity and procedure-related side effects as well as treatment efficacy have been implemented. In Europe, the STOPSTORM consortium aims to collect data regarding efficacy and safety in a large and broad VT patient population treated by multiple centers on various treatment platforms [ 11 ]. In many of these studies, patients with LVAD are excluded.…”
