Steps in the investigation and management of low semen volume in the infertile man

Abstract: An adequate semen volume of ejaculate fluid is required to transport sperm into the female reproductive tract and allow for fertilization of the oocyte. Thus, seminal fluid volume is an important part of the semen analysis done to investigate male infertility. In this article, we review the anatomy and physiology of ejaculation, the various etiologies of low-volume ejaculation (artifactual, structural, functional). We then present a comprehensive algorithm for the evaluation, diagnosis and treatment of the inf… Show more

“…In addition, age-related reduction in sexual intercourse and ejaculation frequency may lead to accessory gland pathologies, i.e. prostatitis and bladder outlet obstruction, reduced fluid secretion from the prostate and seminal vesicles, and therefore to decreased semen volume (25). Similar statistically significant associations between BOO and semen parameters have been revealed in multiple regression analysis.…”

“…A normal volume of semen is needed to promote spermatozoon entry into cervix, buffer the luminal secretions in reproductive tract and maintain sperm activity (Menkveld, ). Inadequate semen volume is one of the causes of male infertility (Roberts & Jarvi, ). The androgen‐dependent epithelia of prostate and seminal vesicles have a key role in fluid transport and can produce a large amount of water and protein‐rich secretions, which consist of most of the ejaculate volume.…”

“…Moreover, the most common reason for RE in patients in infertility clinics is a history of retroperitoneal lymph node dissection (RPLND) for the treatment of testicular cancer. Other major reasons are peripheral neuropathy from diabetes mellitus, surgery (surgery of the bladder neck, transurethral resection of prostate gland), trauma, and idiopathic causes [19,20]. The most useful assessment of RE is evaluation of post-orgasm or postcoital urine samples.…”

“…Semen analysis is the most important examination for the diagnosis of male infertility. An adequate volume of ejaculate is required to carry the male gametes into the female reproductive tract, so ejaculate volume is an essential component of semen analyses [20]. If EjD affects the semen quality of infertile men, then their ejaculate volumes usually decrease or diminish; hence, ejaculate volume should be routinely investigated to detect and treat the low semen volume of EjD-related infertility.…”
