Stent Graft Repair of Traumatic Vertebral Pseudoaneurysm with Arteriovenous Fistula

Abstract: Traumatic vertebral pseudoaneurysm is a rare occurrence. Such case with an enlarging symptomatic vertebral arteriovenous (AV) fistula was successfully treated with a covered endostent. Presentation, diagnosis and different acceptable treatment plans to this condition are further discussed. Stentgrafts can play an important and effective role in therapy of traumatic vertebral pseudoaneurysm with a concomitant AV fistula.

“…Vertebral artery inadvertent punctures with cannulation or catheterization commonly result in arteriovenous fistula, pseudoaneursym, thrombosis and/or dissection. Forty-six cases of VA injury secondary to CVC are reported (9,17) in the literature. But only in three cases who were treated, the parent artery was preserved with stent-graft placement such as in our case (Tab.…”

“…[284583] Duane et al .,[8] reported an ICA dissection during covered stent placement for the treatment of a traumatic pseudoaneurysm. The patient was maintained on anticoagulation therapy, because endovascular treatment was delayed in order to allow healing of the operative site; however, at follow-up the stent was thrombosed.…”

“…For the treatment of vertebral AVF, preservation of the parental artery is important and must be attempted. Endovascular treatment is a minimally invasive treatment modality and does not have the disadvantages of open surgery [5][6][7]. Different types of endovascular approaches, transvenous, transarterial or both with coil or balloon embolization are reported [6,8].…”

We report a case of vertebral AVF in a hemodialysis patient, first detected by the carotid duplex ultrasonography and successfully treated with endovascular therapy.…”
