Statins and Myotoxic Effects Associated With Anti-3-Hydroxy-3-Methylglutaryl-Coenzyme A Reductase Autoantibodies

Abstract: Statins have a variety of myotoxic effects and can trigger the development of inflammatory myopathies or myasthenia gravis (MG) mediated by immunomodulatory properties. Autoantibodies to 3-hydroxy-3-methylglutaryl-coenzyme A reductase (HMGCR) have been identified in patients with statin-associated myopathy. The purpose of the present study is to develop an enzyme-linked immunosorbent assay (ELISA) of anti-HMGCR antibodies and to elucidate the clinical significance of anti-HMGCR antibodies in Japanese patients … Show more

“…This subset usually affects older people and the response to treatment is usually favourable. Subsequently, as several independent groups have investigated the antibodies, the ratio of statin-exposed patients among anti-HMGCR antibody-positive patients has declined from 63–73% to 15–44%, the latter consistent with the result of the present study (18%) 11–13 17 22. The lower frequencies may suggest that production of anti-HMGCR antibodies is not directly related to statin treatment.…”

“…The presence of anti-SRP antibodies may be useful for physicians considering more aggressive immunotherapy when their patients are not responding to corticosteroids. On the other hand, we only established the detection system of anti-HMGCR antibodies in 2014 17. Thus, positivity of anti-HMGCR was not taken into consideration in immunotherapy in our study.…”

“…[11][12][13][14][15] Evidence has emerged of a pathogenic role for anti-HMGCR antibodies, as high levels of these antibodies positively correlate with serum CK value and the severity of muscle weakness. [16] Nevertheless, the mechanism that links autoimmunity to statins has not been entirely clarified by the current hypothesis that statin exposure increases HMGCR expression and may be accompanied by aberrant proteolysis of this molecule.…”
