BackgroundSeveral studies have reported improved survival in severe bacterial infections among statin treated patients. In addition, statins have been ascribed beneficial anti-inflammatory effects. The aim of this study was to evaluate the effect of statin-treatment on mortality in patients with bacterial infections, by means of a systematic review and a meta-analysis.Methodology and Principal FindingsStudies investigating the association between statin use and mortality in patients with bacterial disease were identified in a systematic literature review and a meta-analysis was performed to calculate the overall odds ratio of mortality in statin users. The literature search identified 947 citations from which 40 relevant studies were extracted. In all, 15 studies comprising 113 910 patients were included in the final analysis. Statin use was associated with a significantly (p<0.0001) reduced mortality in patients suffering from bacterial infections (OR 0.52, 95% CI 0.42–0.66). However, all studies included were of observational design and funnel plot analyses indicated influence by a possible publication bias (Egger's bias test p<0.05). When a precision estimate test was used to adjust for publication bias the effect of statin treatment was no longer significant, with an OR of 0.79 (95% CI 0.58–1.07).Conclusion/SignificanceAccording to the meta-analysis of observational studies presented here, patients on statin therapy seem to have a better outcome in bacterial infections. However, the association did not reach statistical significance after adjustment for apparent publication bias. Thus, there is a great need for randomised controlled trials investigating the possible beneficial effect of statins in bacterial infections.

Infectious complications of cardiac surgery are often severe and life threatening. Statins having both immunomodulatory and anti-inflammatory effects were intuitively thought to influence the development of postsurgical infections. We sought to systematically examine whether any association exists between statin use and risk of infectious complications in patients undergoing cardiac surgery. We searched Ovid MEDLINE, Ovid EMBASE, Thomson Scientific Web of Science, and Elsevier Scopus from inception through February 2011 for comparative studies examining the association between statin use and risk of postoperative infections in patients undergoing cardiac surgery. We contacted a study's author for missing information. We conducted a random-effects meta-analysis of individual studies' odds ratios (adjusted for potential confounders). We identified 6 cohort studies for inclusion, 3 of which were conducted in Canada and 3 of which were conducted in the United States. Four were single-center studies, and 2 were population based. Exposure ascertainment was based on a review of admission medication list or prescription databases. Infectious outcomes were heterogeneous and included surgical site infections within 30 days, serious infections (sepsis), or any other postoperative infection. Statin use in the preoperative period was associated with a trend toward reduction in the incidence of postoperative infections in patients who underwent cardiac surgery (odds ratio, 0.81 [95% confidence interval, 0.64-1.01]; [Formula: see text]; [Formula: see text]). Heterogeneity was explained by country effect. Studies performed in Canada showed weaker associations than studies performed in the United States. This difference could not be attributed to study quality alone. We did not find good evidence to support an association between statin use and postoperative infectious complications. However, the trend toward statistical significance for this association indicates that further investigation is warranted.

Este artículo surge de la investigación “Manejo de las heridas quirúrgicas, desde el tipo, técnica e insumos utilizados en una ips. Armenia, Quindío, 2014, financiado por la Vicerrectoría de Investigaciones de la Universidad del Quindío. La investigación describe los métodos que son manejados para el cuidado de las heridas por parte del personal de enfermería que realiza dicho procedimiento, con el fin de conocer con detalle la técnica, o técnicas, y los procesos utilizados en el procedimiento, materiales, pasos, asepsia, entre otros; contrasta lo observado con los avances científicos-tecnológicos en este campo y determina el cuidado de enfermería brindado a los pacientes adultos hospitalizados con heridas de diferente etiología. La investigación fue de tipo Descriptiva-Transversal, con enfoque cuantitativo. La muestra se obtuvo a través de un muestreo intencional, para este caso, pacientes con herida quirúrgica hospitalizados en los servicios quirúrgicos, que ingresaron durante un trimestre por cirugía programada, complicaciones quirúrgicas o por cirugía de urgencia, en una IPS de tercer nivel de atención de Armenia, y quien es eligieron voluntariamente su participación. La técnica para la recolección de la información fue a través de la filmación del procedimiento. Posteriormente, se diligenciaron los instrumentos diseñados para los fines de la investigación; los datos fueron ingresados a Bases de Datos construidas y procesadas en SPSS.
