AbstarctElectrical and structural remodeling processes are contributors to the self-perpetuating nature of atrial fibrillation (AF). However, their correlation has not been clarified. In this study, human atrial tissues from the patients with rheumatic mitral valve disease in either sinus rhythm or persistent AF were analyzed using a combined transcriptomic and proteomic approach. An up-regulation in chloride intracellular channel (CLIC) 1, 4, 5 and a rise in type IV collagen were revealed. Combined with the results from immunohistochemistry and electron microscope analysis, the distribution of type IV collagen and effects of fibrosis on myocyte membrane indicated the possible interaction between CLIC and type IV collagen, confirmed by protein structure prediction and co-immunoprecipitation. These results indicate that CLICs play an important role in the development of atrial fibrillation and that CLICs and structural type IV collagen may interact on each other to promote the development of AF in rheumatic mitral valve disease.

BackgroundPrevious studies regarding rhythm control in patients with atrial fibrillation (AF) could not sufficiently demonstrate the efficacy of available anti-arrhythmic drugs. ‘Upstream therapy’ has emerged as a potential strategy for the prevention and treatment of AF. The use of angiotensin II receptor blockers and statins has been suggested to decrease new-onset AF, but which remains inadequately explored. This study was designed to examine whether valsartan or fluvastatin can reduce the risk of non-permanent AF in patients with hypertension.Methods/designThe VF-HT-AF study is a multicenter, randomized, open-label, four-arm parallel group study with comparative evaluation of valsartan and fluvastatin as upstream therapies for the treatment of non-permanent AF complicated by hypertension. The primary outcome measure is change in the development of paroxysmal AF into persistent or permanent AF, the development of persistent AF to permanent AF, and change in incidence of overall and persistent AF recurrence, as evaluated by 7-days ambulatory electrocardiograph monitoring (Holter) and patients’ diaries during 2 years’ follow-up. Secondary outcome measures of this study include the occurrence of: (1) fatal and nonfatal myocardial infarction; (2) heart failure (New York Heart Association stage III or IV); (3) cardiogenic shock; (4) serious bleeding necessitating hospitalization; (5) malignant ventricular arrhythmia; (6) revascularization therapy; (7) radiofrequency catheter ablation of AF; (8) changes of left atrial dimension, as measured by ultrasound echocardiography; (9) stroke; (10) cardiovascular mortality; and (11) all-cause mortality. A total of 1879 patients will be investigated from 15 medical centers throughout China to obtain the relevant information.DiscussionThis is the first study in hypertensive patients complicated non-permanent AF in the Chinese population. ÖzetAmaç: Atriyal fibrilasyon kalp cerrahisi sonrası en sık görülen ritim bozukluğudur. Perioperatif seyri olumsuz etkilemekte, morbidite ve mortalitede belirgin artışa neden olmaktadır. Kanıtlanmış risk faktörleri yanında, yeni tanımlanmakta olan bir çok risk faktörü de mevcuttur. Hemoglobin A1c diyabetik hastalarda glisemik kontrolün takibinde kullanılan bir belirteçtir. Son yıl-larda atriyal fibrilasyon ile ilişkisi olduğu gösterilmiştir. Biz bu çalışmada, tek merkezde, diyabetik hastalarda preoperatif bakılan hemoglobin A1c seviyeleri ve postoperatif atriyal fibrilasyon arası ilişkiyi göstermeyi amaçladık. Gereç ve Yöntem: Mayıs 2011-Aralık 2011 arası iskemik kalp hastalığının tedavisi amaçlı opere edilen toplam 261 hastanın 109'u diyabetikti (%41.76). Ça-lışmaya dahil edilen bu hastalardan preoperatif hemoglobin A1c seviyeleri bakıldı ve postoperatif dönemde atriyal fibrilasyon gelişimi arasındaki ilişki değerlendirildi. Bulgular: Çalışmaya dahil edilen 109 hastanın yaş ortalaması 62.05 ± 7.85 idi ve hastaların %81.6'sı erkek idi. Postoperatif atriyal fibrilasyon insidansı %19.3 idi. Atrial fibrilasyon gelişen grupta hemoglobin A1c değerleri %8.52 ± 1.96 iken; AF gelişmeyen grupta %8.23 ± 1.52 idi (p>0.05). Hastaların preoperatif demografik özellikleri ve intraoperatif değişkenleri incelendiğinde atriyal fibrilasyon ile anlamlı ilişki saptanmadı. Postoperatif dönemde atriyal fibrilasyon gelişen grupta hastane yatış süresi anlamlı olarak fazla idi (p<0.05). Sonuç: Hemoglobin A1c değerleri ve postoperatif atriyal fibrilasyon arası ilişki nispeten yeni tartışılan bir konu olup, literatürde çe-lişkili sonuçlar mevcuttur. Bizim çalışmamızda hemoglobin A1c değerleri ve postoperatif atriyal fibrilasyon arası anlamlı bir ilişki saptanmadı. Bu konuyla ilgili daha geniş çalışmalara ihtiyaç duyulmaktadır. Hemoglobin A1c is a marker used in glycemic control. It has recently been shown to correlate with occurence of postoperative atrial fibrillation. In this single center study, we aimed to demonstrate the relation between preoperative hemoglobin A1c levels and post-cardiac surgery atrial fibrillation rates in diabetic patients. Material and Method: Between May 2011 and December 2011, 109 diabetic out of 261 patients (41.76%) operated for ischemic heart disease management were included in the study.
