Statin and Its Association With Delirium in the Medical ICU*

Abstract: The use of statins was independently associated with a reduction in the risk of delirium in hospitalized patients. When considering types of statins used, this reduction was significant in patients using atorvastatin, pravastatin, and simvastatin. Randomized trials of various statin types in hospitalized patients prone to delirium should validate their use in protection from delirium.

“…Our study reported that the incidence of postoperative delirium was 13.5% of placebo patients, and 5.6% in the rosuvastatin group, The results were similar to a large number of the previous studies [11,15,[18][19][20]. Liu [21] reported a retrospective cohort study that included 361 elderly individuals who underwent noncardiac surgery, there were 19.9% individuals developed postoperative delirium after surgery.…”

“…Morandi et al [ 23 ] observed that statin for critically ill patients was associated with reduced delirium, despite of a short statin usage period, especially during ICU stay. Similarly, Mather et al [ 24 ] reported the favor effect of statin for delirium in the critically ill patients. Our study also showed a favorable effect of statin, and similar risk factors for delirium, such as age, higher ASA, emergent operation, and prolonged operation, likewise other studies.…”

“…Three cohort studies suggest that when statin use is stopped during critical illness, delirium occurrence increases (371)(372)(373). However, one recent randomized study of delirium-free cardiac surgery patients admitted to the ICU (not included in the evidence profile for this question) found that the use of preoperative atorvastatin did not affect incident delirium (374).…”
