State-Level Cardiovascular Mortality Rates Among Hispanic, Non-Hispanic Black, and Non-Hispanic White Populations, 1990 to 2019

Abstract: ImportanceCardiovascular disease (CVD) is the leading cause of death in the US, with considerable variation by both state and race and ethnicity group. Consistent, comparable measures of mortality by specific CVD cause at the state level and by race and ethnicity have not previously been available and are necessary for supporting policy decisions aimed at reducing health inequities.ObjectiveTo quantify and describe levels and trends of mortality due to overall CVD and its component causes for 3 mutually exclus… Show more

“…95% confidence intervals are in parentheses. *** p<0.001, ** p<0.01, * p<0.05 c Predicted outpatient pharmacy expenditures are subset of total outpatient expenditures reported in column (2). Outpatient pharmacy expenditure estimates were obtained using GLM regression with gamma distribution and loglink for fully-adjusted model with overlap propensity score-weights among those who had positive outpatient pharmacy expenditures during 2021.…”

“…Cardiovascular diseases (CVD) are leading causes of death in the United States (US); 1,2 and hypertension is one of the major modifiable risk factors for CVD. 3 In 2021, hypertension was a primary or contributing cause of 691,095 deaths in the US 4,5 , and, according to prior projections, it is also one of the fastest-growing cardiovascular causes of spending.…”

“…Although overall mortality rates and those attributable to cardiovascular disease (CVD) and cancer steadily declined in the United States (US) between 1990 and 2019, significant health disparities persist by race/ ethnicity, socioeconomic status (SES), and geographic region, with higher mortality rates among Black or African American adults compared with non-Hispanic White adults [1][2][3]. Society and community-level factors are fundamental drivers of health disparities [3][4][5].…”

“…18 They build on that literature by estimating effects by race and ethnicity, an important endeavor given people of color are more likely to have their eligibility for Medicaid affected by Medicaid expansion, 19 and Black individuals in particular bear a persistently high burden of CVD mortality.
