Standardized Uptake Value and Resection Margin Involvement Predict Outcomes in pN0 Head and Neck Cancer

Abstract: A median (18)F-FDG PET/CT SUVmax cutoff values of 8.5 or greater and positive surgical margins were associated with adverse outcomes in patients with pN0 head and neck cancer.

“…27 Multiple prior reports demonstrate that positive margins independently increase the risk of disease recurrence and decrease survival in early OCSCC. 5,6,[10][11][12][13] Our data show that margin status is well reported and easily measured, with 95% of our sample having a documented margin status and a median rate of margin documentation among reporting institutions of 100%. This is consistent with 1 prior retrospective study of tongue cancer from the Texas M. D. Anderson Cancer Center, which found the institutional rate of margin status documentation to be similarly high at 97.4%.…”

“…10 Also, some authors used the maximum standard uptake value to predict outcomes. [11][12][13][14] Not surprising, as a result of all these advantages, the use of PET/CT has increased significantly over the last several years. 15 Despite the advantages of PET, there are important limitations of this imaging modality.…”

“…It has become common practice to obtain PET/CT for evaluation of HNSCC of unknown primary to aid in identification of the primary site 10 . Also, some authors used the maximum standard uptake value to predict outcomes 11‐14 . Not surprising, as a result of all these advantages, the use of PET/CT has increased significantly over the last several years 15…”

“…The principle of surgery with curative intent is based on achieving an adequate disease‐free resection margin. Close or positive resection margins are associated with a significantly higher risk of recurrent disease . Resection with adequate margins may be limited by surrounding essential structures, reconstruction capabilities, high risk of morbidity, or dysfunction .…”

“…In these circumstances, histology is required. Joo et al, Dillon et al, and Heiduschka et al, among others, showed that not only tumor‐positive resection margins are of influence on recurrence rate and outcome. Other tumor characteristics such as perineural growth, tumor thickness, and extracapsular nodal spread are important prognostic factors.…”
