Aphasia, impairment of language after stroke or other neurological insult, is a common and often devastating condition that affects nearly every social activity and interaction. Behavioral speech and language therapy is the mainstay of treatment, although other interventions have been introduced to augment the effects of the behavioral therapy. In this narrative review, we discuss advances in aphasia therapy in the last 5 years and focus primarily on properly powered, randomized, controlled trials of both behavioral therapies and interventions to augment therapy for post-stroke aphasia. These trials include evaluation of behavioral therapies and computer-delivered language therapies. We also discuss outcome prediction trials as well as interventional trials that have employed noninvasive brain stimulation, or medications to augment language therapy. Supported by evidence from Phase III trials and large meta-analyses, it is now generally accepted that aphasia therapy can improve language processing for many patients. Not all patients respond similarly to aphasia therapy with the most severe patients being the least likely responders. Nevertheless, it is imperative that all patients, regardless of severity, receive aphasia management focused on direct therapy of language deficits, counseling, or both. Emerging evidence from Phase II trials suggests transcranial brain stimulation is a promising method to boost aphasia therapy outcomes.

Purpose The purpose of this article is to examine the current state of counseling curriculum within the discipline. The last systematic survey of counseling curriculum within the disciplines of communication sciences and disorders was completed with data from 1983 (McCarthy et al., 1986). The Council on Academic Accreditation in Audiology and Speech-Language Pathology (2017) states that counseling should be included in accredited programs but does not specify to what extent. Currently, there are no standards to specify number of credits, need for a stand-alone course, or guidance regarding content delivered. Method The present investigation collected data on the status of counseling curricula in accredited communication sciences and disorders graduate programs. A Qualtrics survey was distributed to identify counseling curriculum practices across accredited programs. Quantitative data such as percentages and frequency counts were compiled to summarize program offerings. Qualitative analyses were used to characterize written responses. Survey responses were also cross-validated with a review of offerings listed on program websites. Results Of programs currently accredited by the Council on Academic Accreditation in Audiology and Speech-Language Pathology, 42.4% responded to the current survey. Fifty-nine percent of programs offer a stand-alone course. Review of curricula from program websites indicated that only 40% of accredited programs offer a stand-alone counseling course. Quantitative details about requirements, number of credits, and embedding counseling within other courses were compared to data from the 1983 survey. Qualitative analyses identified common learner outcomes and the nature of course or curricular content. Conclusions Investigators found a lack of consistency in incorporating counseling across programs and discussed implications of this in speech-language pathology practice. A decrease in the number of programs that offer a stand-alone counseling course was identified as compared to offerings in 1983, as well as a disparity regarding how programs provide training in counseling. Furthermore, survey responses differed from curriculum listings on program websites. Information derived from this study may serve as a starting point for the development of flexible standards that provide direction for achieving consistent preparation of counseling skills. Contribution of acoustic analysis to the detection of vocoid epenthesis in apraxia of speech and other motor speech disorders

Background: Vocoid epenthesis within consonant clusters has been claimed to contribute to the diagnosis of apraxia of speech. In clinical practice, the clinicians often doubt about the correct production of clusters as the C-C transition may be minimally disrupted. Aims: To demonstrate the value of acoustic analysis in clinical practice as a reliable complement to perceptive judgment. Methods & Procedures:We compared the acoustic signature and the perceptive detection of vocoid epentheses in unvoiced consonant clusters within pseudo-words produced by 40 participants presenting different subtypes of motor speech disorders (including apraxia of speech (AoS) and dysarthria) and matched neurotypical controls. Outcomes & Results:The results indicate that vocoid epenthesis was acoustically visible in 3 out of 10 participants with AoS, and in one out of 30 participants with dysarthria. One-quarter of these vocoid epentheses was not detected via auditory perception by expert listeners (speech and language therapists) who also made false detections. Conclusions:The current results indicate that vocoid epenthesis is not systematic at least in mild AoS. Moreover, an important proportion is misdetected by ear, even by expert clinicians, meaning that visualisation of the acoustic signal can be of precious help.
