Stability of Measures of Pain Catastrophizing and Widespread Pain Following Total Knee Replacement

Abstract: Objective. Pain catastrophizing and widespread pain are predictors of pain chronicity/severity. Gaps remain in our understanding of the extent to which each is a stable (trait) or dynamic (state) variable. We undertook this study to assess the stability of each variable from before to after total knee replacement (TKR) and whether changes are explained by pain improvements.Methods. We used data from a prospective study of TKR recipients ages ≥40 years. Questionnaires included body pain diagrams assessing wides… Show more

“…Longitudinal studies among people with related conditions such as chronic widespread pain (CWP) or temporomandibular disorder suggest that psychological factors are weak predictors of future pain compared to factors such as somatic amplification, interoception, prior pain, poor sleep and smoking 2,3 . Furthermore, numerous studies suggest that psychological factors often improve dramatically when pain improves -which would not be expected if this unidirectional hypothesis is correct 4,5 .…”

“…While catastrophizing has often been viewed as a personality trait that remains stable in the absence of intervention [e.g., 58 , 59 ], other findings that catastrophizing decreased following pain relief, suggest dynamic, state-like aspects [ 60 ]. Thus, the PCS, may comprise both situational and dispositional elements which are affected, among other reasons, by the instructions provided (whether relate to current or on previous pain experience) [ 61 ]. Furthermore, Sullivan et al [ 58 ] proposed that the extent to which one engages in catastrophizing might change over time as a function of stimulus cues and social responses present in the individual’s environment.…”

“…Many early studies in these conditions focused on these psychological factors being causative, since many or most individuals with a nociplastic pain condition will exhibit some form of psychological distress. But more recent studies show that many individuals with chronic pain who have high levels of depression or catastrophising have rapid improvements in these domains when they are successfully treated for their pain,23 24 suggesting a strong bidirectional relationship. When a nociplastic state is triggered by a stressor such as deployment to war, infections or trauma the typical course is that individuals begin sleeping poorly, become fatigued and less active, and develop distress since they are uncertain of whether or when they will improve.…”
