Squamous cell carcinoma with rhabdoid phenotype and osteoclast-like giant cells in a renal–pancreas transplant recipient

Abstract: Squamous cell carcinoma (SCC) is the commonest non-melanotic malignant skin tumour encountered after solid-organ transplantation. In this setting it is associated with a worse prognosis than sun-damage-induced SCC. Rhabdoid cells and osteoclastic giant cells are infrequently seen in SCC. This case highlights the unusual occurrence of rhabdoid cells and osteoclastic giant cells in a post-transplant SCC.

“…The observation to date has been made in the context of malignant melanoma, 5 squamous cell carcinoma, 6,7 and an adnexal carcinoma. 8 In each of the carcinomas, the giant cells were associated exclusively with sarcomatoid elements of the neoplasm.…”

“…Cutaneous squamous cell carcinoma (SCC) associated with osteoclast‐like giant cells (osteoclastic cells) is very rare; only eight cases have been reported since 2006 . In the skin, stromal changes containing osteoclasts can occur in SCC, melanoma, basal cell carcinoma, nodular fasciitis, dermatofibroma, giant cell tumor of tendon sheath, giant cell malignant fibrous histiocytoma, atypical fibroxanthoma, phosphaturic mesenchymal tumor, soft tissue giant cell tumor of low malignant potential and pleomorphic dermal sarcoma.…”

“…To the best of our knowledge, this is only the seventh reported case of squamous cell carcinoma with rhabdoid features (Table I). Of interest, all occurred in older male patients; patients also had a history of kidney failure and our patient was the second with history of multiple transplantations 5–9…”

“…Squamous cell carcinoma with rhabdoid features is an extremely rare variant of squamous cell carcinoma and has a high potential for aggressive behavior and a poor outcome. Although the diagnoses of such lesions have been documented in the surgical pathology literature,5–9 reports in the cytology literature however have not yet appeared. To the best of our knowledge, it is the first fine needle aspiration (FNA) cytology case report of this entity and the second case in a patient with two autologous organ transplantations 5–9.…”
