“…Primary metastasis to the flap donor site is a rare complication of flap reconstruction, first reported in 1981 following cross‐leg flap for reconstruction of a fibrosarcoma resection defect 5 . Seeding of the donor site by head and neck SCCA has been described following use of a split thickness skin graft 6 and a variety of reconstructive flaps 7–11 . This case is the first report of head and neck primary SCCA recurrence at an SAI flap donor site.…”

“…Lastly, tumor metastasis can occur via implantation either by spontaneous seeding or by iatrogenic contamination during surgery 7 . Contaminated instruments or equipment has been proposed as a probable cause of donor site seeding in several case reports 8,9 and is the most likely mechanism of recurrence in this patient. This theory is further supported by the lack of other metastatic disease, which would be expected with lymphatic or hematogenous spread.…”

“…We consider that the PMMF may not be large enough to allow reconstruction of major through‐and‐through defects and may not be long enough for application to the skull base. Additionally, accidental displacement of a pectoralis major flap by a breast tissue adenocarcinoma and SCC developing in the PMMF donor site have been reported . A much higher proportion of female patients received TIMFs (52.4%) than PMMFs (16.5%); use of a TIMF is preferred in women when myocutaneous flaps are not ideal.…”
