Spurious Macrocytosis, A Common Clue to Erythrocyte Cold Agglutinins

Abstract: In four patients with cold agglutinins, erythrocyte mean corpuscular volume was increased. Erythrocyte volume histograms showed that in each case macrocytosis was due entirely to doublet erythrocytes counted as single cells. For two of the four patients, macrocytosis was reported intermittently, when the blood had been allowed to cool to room temperature during laboratory processing. For these two patients, macrocytosis due to doublet erythrocytes could be elicited in vitro proportionate to the degree of cooli… Show more

“…Both of these findings reverse after the blood sample is incubated at 37°C for 20 min and are reanalyzed. 10,13 In addition to the red cell cytograms presented in this study, those with intermediate or indeterminate patterns are also seen in daily practice. An attempt must be made to optimally use the ones pointing to a particular hematological condition while maintaining a database of others for pattern matching.…”

“…[12] Cold agglutinins make RBCs clump, making them appear larger to automatic cell counters. [13] Non Hodgkin Lymphomas may be associated with cold agglutinins resulting in spurious macrocytosis. [14] Weiss and Bessman reports two patients with warm autoimmune HA whose MCV values where spuriously high.…”

“…Similarly, in our clinical observations, a decrease in rates of relapse could not be shown in cases in whom mucositis which is a side effect of methotrexate had a severe prognosis. Recently, White et al (11) investigated if increased MCV was a marker for hematologic toxicity in patients receiving methotrexate treatment with a diagnosis of rheumatic arthritis, but they could not find a relation . In another study, the relation of MCV values with cytogenetic response and prognosis was investigated in patients with a diagnosis of chronic myeloid leukemia receiving imatinib treatment (13).…”
